RN CASE MANAGER

Our elite team of acute care professionals treats medically complex patients requiring extended acute care treatment. Most patients are ventilator-dependent with multiple co-morbidities. The RN Case Manager position works with the Partner hospital/Insurers/Clinical Liaisons and Unit Leadership to identify and approve potential patients who would benefit from services provided by the TriVent Unit. This position provides comprehensive assessment, planning, implementation and overall evaluation & coordination of individual patient needs from the preadmission process through discharge. This position must fully understand the admission criteria of the Unit and be capable of educating others on such criteria. This position must be able to effectively communicate with hospital personnel, Unit leadership, physicians and the interdisciplinary teams to develop a plan of care for each assigned patient from admission through discharge and be able to effectively communicate this plan of care to the patient’s family. This role is key to ensuring that the patient is progressing towards the desired outcome by continuously monitoring patient results through daily assessments and/or evaluations.

Education/Experience:
- Education – Bachelor’s degree in Nursing,
- Experience – A minimum of one year prior acute care experience in Case Management in a hospital setting.
- Licensure – Active State Registered Nurse license
- Certifications – BLS required
