Job Description
The RN Case Manager provides patient care management across the continuum of care. The RN Case Manager is responsible for ongoing assessment of patient needs and coordination of the delivery of patient care through the unit manager, Social Worker, and the multi-disciplinary health care team. The RN Case Manager is accountable to ensure patients meet criteria for admission, continued stay, and discharge. They focus on facilitating the development of discharge plans and the appropriate movement of patients from one level of care to another. In collaboration with the Social Workers, they insure a smooth transition to other levels of care. The RN Case Managers function as a liaison with insurers to advocate for patients based on defined needs. There is a significant focus on patient care outcomes and the RN Case Manager oversees the evaluation of outcomes (treatment and financial outcomes). The practice of the RN Case Manager is highly collaborative with physicians and other health care team members. They are involved in quality improvement utilizing trended data to focus on issues identified for improvement. RN Case Managers are teachers to patients/families and members of the multi-disciplinary team. RN Case Managers are accountable for the training related to the principles of care and case management. They assist the Social with the complicated medical issues.

Requirements:
Education 2 year / associate degree (preferred) License / Certification Valid OK RN License BLS Certified Case Manager (preferred) Experience 1 - 3 years

* Instruct individuals, families, or other groups on topics such as health education, disease prevention, or childbirth and develop health improvement programs.
* Modify patient treatment plans as indicated by patients' responses and conditions.
* Observe nurses and visit patients to ensure proper nursing care.
* Assess the needs of individuals, families, or communities, including assessment of individuals' home or work environments, to identify potential health or safety problems.
* Work with individuals, groups, or families to plan or implement programs designed to improve the overall health of communities.
* Prepare patients for and assist with examinations or treatments.
* Order, interpret, and evaluate diagnostic tests to identify and assess patient's condition.
* Prescribe or recommend drugs, medical devices, or other forms of treatment, such as physical therapy, inhalation therapy, or related therapeutic procedures.
* Administer local, inhalation, intravenous, or other anesthetics.
