Job Description
Position Summary
Under the direction of the Director of Clinical Services, the Registered Nurse or Licensed Practical Nurse position supports the Mission, Value and Vision of Advance Community Health Center to deliver quality care to all our patients, support health and assists with care provided by Medical Assistants. The Clinic Nurse RN/LPN will assist the medical providers to provide comprehensive medical care to patients, support staff in the clinic setting and perform a range of activities and tasks with the patients. The RN or LPN will help facilitate conversations between interdisciplinary Care Teams and expedite patient care. Coordinates nursing and medical interventions, patient care planning, education, promotes optimal patient care. We have a need at our Wake County & Franklin County locations.
Essential Duties and Responsibilities
- Conducts an individualized patient assessment, prioritizing the data collection based on the patient’s immediate condition or needs within timeframe specified by client facility’s policies, procedures or protocols.
- Provide patient care/Clinical Tasks within RN/LPN scope of practice and as directed by medical providers.
- Provide patient and family education.
- Responsible for the triage of messages and obtaining appropriate information from patient and relaying to provider in the EHR (EPIC)
- Immunization Program Monitoring.
- Provide clinical oversight for Medical Assistants and monitor competencies.
- Monitor Nurse/MA Clinic Schedules.
- Assist in conducting Medicare Annual Wellness Visits.
- Remain up to date with relevant medical information regarding population service.
- Serve as a role model for other staff, setting a professional tone for the clinic.
- Assist with prescriptions/patient paperwork/lab results and follow up calls as directed by medical providers.
Other Duties Assigned
- Perform other duties as assigned or become necessary by supervisor.
Supervisory Responsibility
- Provide training to clinical support.
Education and Experience
- Associate degree or four-year degree in Nursing program; North Carolina (RN or LPN) license required.
- Minimum 2 years’ experience in an outpatient setting or public health center required.

Work Schedule: Monday – Friday, 8:00an – 5:00pm with flexibility (No Remote Option at this time)
