We are looking to grow our Home Care Team!

Care Around the Block is a local and family-owned company that provides Care Management to older adults, partnering with them in their aging journey. Our objective is to assist our clients and their responsible parties in managing and navigating the issues they will face as they age through the process of assessment, care planning, and evaluation of care.

Serving Knox, Anderson, Blount, Loudon, and Monroe counties.

Job Summary: The successful Care Manager gets a “rush” when she/he walks into a seemingly hopeless situation and can help the client and family meet their goals. They are fulfilled by knowing a job is well done. You would be responsible for providing quality professional care management services to all clients and their responsible parties adhering to the ALCA Pledge of Ethics and Standards of Practice and Care Around the Block’s mission statement.

Classification: Non- Exempt, Hourly. Part-time positions available.

Care Manager Responsibilities:

  • Advocate for clients
  • Conduct care plan assessments. The assessment may include medical history, current level of functioning, cognitive and emotional status, safety of the environment, as well as any financial or legal needs.
  • Develop a comprehensive plan of care that includes recommendations:
    • Implement, monitor, manage and update the care plan.
    • Offer referrals to medical, financial, or legal specialists.
  • Ensure there are no inappropriate placements, duplication of services, and unnecessary hospitalizations.
  • Assist with transition to an appropriate facility/program as needed.
  • Coordinate communication between all physicians, clients, and their responsible family.
  • Ongoing monitoring of clients’ health status and well-being.
  • Provide crisis intervention as needed.
  • Provide client/consumer education as needed.
  • Complete documentation requirements accurately, efficiently, and effectively within 24 hours.
  • Comply consistently with established time standards and deadlines.
  • Participate in Care Management On-Call Rotation
  • Complete additional tasks as requested by supervisor.

Care Manager Qualifications:

  • TN Licensed Registered Nurse (Associates or Higher required/ BSN preferred).
  • CCM Certification preferred.
  • Minimum of 3 years of experience in local human service or medical field, as well as community resources regularly utilized by older adults (i.e. Medicare, Medicaid, Community resources, etc.).
  • Reliable transportation and vehicle insurance.
  • Care Manager office duties will be performed from home. Must have high speed internet access and ability to work in a private environment for HIPAA compliance.
  • Computer proficiency in Microsoft Word, Excel, and Outlook. Must be technologically adept and able to learn new software.
  • Ability to pass a Criminal Background Check, Drug Screen, and DMV Check at hire and randomly as required by company policy. Maintain insurability per company policy.

Work Environment: This job operates in a professional office environment and extends into clients’ homes. May be required to perform certain duties from a home office.

Travel: Primarily local during the business day, although some out- of- area and overnight travel may be expected.

Physical Demands: Must be able to sit for long periods, stoop, lift, and bend effectively to be able to perform the above-listed Job Duties & Responsibilities. Must be able to lift 25 pounds. Must be able to sit in a car and drive, sometimes for several hours per day.
