Care Around the Block is currently seeking a Social Worker to join our growing professional care management team! You will be responsible for providing quality professional care management services to all clients and their responsible parties adhering to the ALCA (Aging Life Care Association) Pledge of Ethics and Standards of Practice and the Company’s mission statements. Care management is defined as professional consulting that assists clients and their responsible parties with decisions related to aging, available services, and long-term care planning.

Who We Are
Care Around the Block is a local and family-owned company that provides Care Management to older adults, partnering with them in their aging journey. Our objective is to assist our clients and their responsible parties in managing and navigating the issues they will face as they age through the process of assessment, care planning, and evaluation of care.
Responsibilities
- Act as an advocate for clients.
- Conduct care plan assessments to identify problems, eligibility for assistance, and need for services. The assessment may include medical history, current level of functioning, cognitive and emotional status, environment safety, and any financial or legal needs.
- Develop a plan of care that includes recommendations to increase client safety and enhance their quality of life and monitor all services to optimize client benefits. Implement, monitor, manage and update the care plan.
- Offer referrals to medical, financial, or legal specialists.
- Ensure there are no inappropriate placements, duplication of services, and unnecessary hospitalizations.
- Assist with transition to an appropriate facility/program and ensure member is in the correct program.
- Act as a liaison and ensure coordination of communication between all physicians, clients, and their responsible family. Communication must be completed within 24 hours.
- Performs all activities related to contracted services per contracted intervention protocol to achieve program goal utilizing tele-health platform/software and proprietary data management system as assigned.
- Responsible for assisting clients and their responsible parties in managing and navigating the issues they will face in the healthcare community and connecting them with resources.
- Participate in Care Management On-Call Rotation
- Complete additional tasks as requested by supervisor.
- Responsible for adhering to company mission, philosophies and all policies and procedures.
- Responsible for professional behavior on behalf of clients and in all community venues. Promote and represent the Company whenever an opportunity arises.
Requirements
- TN Licensed master's level Social Worker, LCSW preferred.
- Minimum of 3 years of experience in local human service or medical field, and resources regularly used by older adults (i.e., Medicare, Medicaid, Community resources, etc.) in your community.
- Reliable transportation and vehicle insurance.
- As many of the Care Manager duties will be performed from a home office, must have high-speed internet access and the ability to work in a private environment for HIPAA (Health Insurance Portability & Accountability Act) compliance.
- Computer proficiency in Microsoft Word, Excel, and Outlook. Must be technologically adept and able to learn new software.
- Ability to pass a Criminal Background Check, Drug Screen, and DMV Check at hire and randomly as required by company policy. Maintain insurability per company policy.
