Harbor Health operates two Elder Service Plan Programs of All-Inclusive Care for the Elderly (PACE), a program that allows frail elders to live in their homes and communities.  Our Elder Service Plan has two medical and day centers in Mattapan and Brockton, MA and cares for more than 580 participants.

Role: 

The Behavioral Health Clinician provides direct psychotherapy services to Harbor Health’s Program of All Inclusive Care of the Elderly (PACE) participants. The Behavioral Health Clinician assists the Psychiatric Nurse Practitioners, Primary Care Providers, and Social Workers in Behavioral Health functions of the department, including escalation support, screening clients for psychiatric services, diagnosing as appropriate, and engaging with the Interdisciplinary Team (IDT). This clinician must have a passion for providing quality behavioral health services to an underserved and complex geriatric population.

Responsibilities: 

  • Assesses, treats, and evaluates behavioral health conditions with evidence-based therapeutic modalities. 
  • Collaborates with the IDT and psych NPs for treatment and evaluation of progress; coordinate case conferences with other providers, clinicians, family members, and caregivers as appropriate.
  • Conducts ongoing care to both individuals and groups and facilitates individual counseling sessions in accordance with the individualized treatment plan. 
  • Conducts treatment sessions in Adult Day Center/clinic, in community settings, and via telehealth.
  • Refers community resource needs to the Social Work team.
  • Makes referrals as appropriate to PACE Psychiatric Nurse Practitioners.
  • Completes clinical documentation in Electronic Health Record for each client including, but not limited to intake assessments, progress notes, treatment plans, termination summaries, and other documentation in a timely fashion.
  • Manages crisis intervention, triage, and stabilization as urgent clinical situations arise. 
  • Maintains a volume target 20 – 25 patients/week, including episodic/same-day appointments for crisis management.
  • Demonstrates familiarity with the DSM-V and comfortable with diagnosing.
  • Conducts periodic re-assessments with Social Workers and/or Psychiatric NPs to determine ongoing treatment plan, tracks number of sessions and develops transition plan as completion of series nears and effectively transitions participant following treatment completion.
  • Provides timely and relevant communications to IDT, particularly surrounding transitions of care.
  • Manages own schedule in the Electronic Health Record, including booking appointments, submitting transportation requests, and cancelling/rescheduling as needed.
  • During emergency operations may be required to report to work or remain at work.

Requirements: 

  • Master's level degree in relevant discipline or above
  • Current independent licensure required - LICSW, LMHC, and PhD or PsyD in the Commonwealth of Massachusetts in good standing, current BLS (Basic Life Support for Healthcare Providers)
  • Minimum of 3 years of experience providing direct psychotherapy services
  • May substitute LCSW with additional 2 years of psychotherapy services for LICSW/LHMC. 
  • Experience with treating substance use disorders preferred.
  • Experience with high-risk populations.
  • 1 year of experience working with a frail elder population (paid or volunteer experience) preferred.
  • Excellent oral and written communication skills
  • Knowledge of community resources.
  • Basic skills in effectively using Microsoft Office 365 including Word, Excel, Outlook and Teams
  • Critical thinking skills to problem solve individually and in a team setting.
  • Bilingual desired: Spanish, Haitian Creole, and/or Portuguese.
  • Ability to work independently and cooperatively as part of a multidisciplinary team.
  • Ability to prioritize, organize and schedule busy workdays.
  • Must be able to travel within the Mattapan and Brockton territories and Harbor Health ESP locations in a timely manner
