GENERAL SUMMARY: Plan, organize, monitor, and carry out Antimicrobial Management and Stewardship programs to optimize clinical outcomes, promote cost effective therapy and improve care while minimizing unintended effects of infectious disease care in the hospital. Integrate Antimicrobial Management and Stewardship Programs with patient safety and quality improvement initiatives as outlined by Medicare, the Joint Commission, and other agencies where appropriate antimicrobial use is monitored.

These duties are to be performed in a highly confidential manner, in accordance with the mission, values and behaviors of Insight Hospital and Medical Center. DESCRIPTION:

Duties and Responsibilities:
  • Maintenance and dissemination of drug information pertaining to infectious disease, but not limited to, in-service education of hospital personnel, consultation with patients, hospital staff, medical staff, and conferences.
  • Exercises professional judgement in imparting therapeutic guidance and drug information to hospital staff (i.e. pharmacy personnel, medical staff, and nursing staff).
  • Is competent in the range of treatment needed by the following patient as appropriate to the ages (Adult, pediatric, geriatric, adolescent, and psychiatric).
  • Inspects pharmacy records to monitor drug therapy for appropriateness and safety consistent with a high level of patient care.
  • Performs pharmacokinetics, IV to PO conversions, therapeutic interchanges, and other clinical pharmacy functions accurately and in a timely manner.
  • Assists in developing and maintaining reports and information for pharmacy management, hospital utilization, and antimicrobial stewardship team. Annual evaluation and report to included, but not limited to: Antimicrobial utilization trend Hospital Length of Stay trend Antibiogram & Resistance trend Core Measure Data pertaining to antibiotic stewardship management.
  • Serves as a consultant to hospital Pharmacy & Therapeutics committee and other hospital sponsored clinical and performance improvement committees.
  • Is active in clinical rounding, public speaking and presenting technical and financial data using PowerPoint presentation skills or similar format as assigned.
  • Is involved in the development, implementation, evaluation and continuous evolution of clinical pathways in infectious disease management programs.
  • Integrate pharmacy, infection control, information system, and infectious disease medical staff in carrying out antimicrobial management program.
  • Evaluating and maintaining quality initiatives by accrediting organizations pertaining to antimicrobial stewardship program.
  • Evaluating and maintaining quality initiatives by accrediting organizations pertaining to antimicrobial programs.
  • Reports daily to Clinical Pharmacy Manager on overall operations, projects assigned, and expectations of overall role.
  • Rotating shifts AM&PM.


JOB SPECIFICATIONS:
Required Knowledge, Skills and Abilities:

Knowledge:
  • Pharm.D. or equivalent from an ACPE- accredited School of Pharmacy and a current pharmacist license in good standing with appropriate State Board of Pharmacy. Residency in an ASHP accredited program and/or Infectious Disease Fellowship Recommended. Advanced Infectious Disease certification in Antimicrobial Stewardship Recommended.


Skills:
  • Thorough knowledge of state and federal laws and regulations governing pharmacy services. Proficient in the use and application of new medications and technology. Effective verbal and written communication skills are required. Ability to effectively participate on an interdisciplinary team or committee. Knowledge of pathophysiology, interpretation of diagnostic criteria, pharmacokinetics and pharmacotherapeutics . Problems solving ability and analytical thinking preferred.


Abilities:
  • Ability to work independently and handle stress appropriately. Ability to work cooperatively with other members of Mercy Hospital and Medical Center and pharmacy staff. Ability to handle multiple tasks, and adapt to changes in workload and work schedule. Ability to set priorities, makes critical decisions, and respond to client concerns. Fosters a team environment. Ability to communicate effectively (verbal and written).
