GENERAL SUMMARY: The Internal Medicine Clinical Pharmacy Specialist evaluates and interprets orders, compounds; and dispenses medications according to the written orders/prescriptions issued by an authorized prescriber. The clinical specialist will also identify, prevent, assess, and resolve medication-related problems to ensure a safe and optimal patient outcome. The clinical specialist will promote, through criteria-based protocols, clinically rational drug therapy and sound pharmaceutical care.

DESCRIPTION:
Duties and Responsibilities:
- Participates in daily patient care rounds. Monitors drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, and therapeutic duplication and drug interactions. Discuss medication order clarifications with the prescriber, documents all changes in patient and pharmacy records
- Maintain and update patient profiles with demographics, allergies, and current medications on a daily basis. Promote the use of the formulary by converting non-formulary orders to formulary, when possible, but coordinates procurement of non-formulary drugs when necessary.
- Is competent in the range of treatment needed by the following patient as appropriate to the ages (Adult, pediatric, geriatric, adolescent, and psychiatric).
- Inspects pharmacy records to monitor drug therapy for appropriateness and safety consistent with a high level of patient care. Prevent detect, monitor, document, and report adverse drug reactions and medication errors.
- Performs pharmacokinetics, IV to PO conversions, therapeutic interchanges, and other clinical pharmacy functions accurately and in a timely manner.
- Promotes safe, efficacious, and cost-effective medication therapy by participating in the provision and documentation of clinical pharmacy services and execution of medication utilization initiatives (e.g., IV to PO, renal dosing). Responds to medication related inquiries from other health care professionals and patients. Document clinical interventions and cost avoidance as part of the departmental documentation program.
- Serves as a consultant to hospital Pharmacy & Therapeutics committee and other hospital sponsored clinical and performance improvement committees.
- Is active in clinical rounding, public speaking and presenting technical and financial data using PowerPoint presentation skills or similar format as assigned. Contribute to newsletter articles, in-services and educational material for pharmacy staff and other healthcare professionals to promote pharmacy as a respected source of drug therapy information and disease management resource.
- Participate in the planning and implementation of system-wide pharmaceutical care programs to optimize drug therapy outcomes. Complete one clinical project per fiscal year. Project to be determined in collaboration with pharmacy clinical team.
- Ensures all patient care provided meets or exceeds all regulations, statutes, and HFAP standards.
- Reports daily to Clinical Pharmacy Manager on overall operations, projects assigned, and expectations of overall role.
- Counsel's patients on discharge medications (i.e., warfarin education as necessary) and coordinate meds to beds and interventions with Outpatient Pharmacy when needed.
- Rotating shifts AM&PM.
JOB SPECIFICATIONS:
Required Knowledge , Skills and Abilities:
Knowledge:
- Pharm.D. or equivalent from an ACPE- accredited School of Pharmacy and a current pharmacist license in good standing with appropriate State Board of Pharmacy. Residency in an ASHP accredited program preferred. Pharmacotherapy board certification recommended. Experience using computer applications including word processing, spreadsheets, and pharmacy information systems.
Skills:
- Thorough knowledge of state and federal laws and regulations governing pharmacy services. Proficient in the use and application of new medications and technology. Effective verbal and written communication skills are required. Ability to effectively participate on an interdisciplinary team or committee. Knowledge of pathophysiology, interpretation of diagnostic criteria, pharmacokinetics and pharmacotherapeutics. Problem solving ability and analytical thinking preferred.
Abilities:
- Ability to work independently and handle stress appropriately. Ability to work cooperatively with other members of Mercy Hospital and Medical Center and pharmacy staff. Ability to handle multiple tasks and adapt to changes in workload and work schedule. Ability to set priorities, make critical decisions, and responds to client concerns. Fosters a team environment. Ability to communicate effectively (verbal and written).
