About Your Work

The Care Manager - Social Worker is responsible for partnering with external providers across the continuum of care to enhance the overall quality of care delivered to members. Responsible for identifying, developing, coordinating, promoting, supporting, and implementing assigned clinical quality, and/or service improvement projects and programs, the Care Manager acts as a resource for evidence-based practice integration, provider training/education, policy review/development, and regulatory/accreditation interpretation. Manages multiple complex initiatives that impact the quality or effectiveness of health care delivery and/or health care services. Acts in conjunction with both external facing and plan quality improvement leaders and key stakeholders to provide support for multi-disciplinary teams.

Our Supportive Care Program provides that extra layer of care to patients living with a serious illness, incorporating Palliative Care principles to allow care early in the serious illness process. The Supportive Care Program Social Worker plays a critical role in providing comprehensive and compassionate care to patients facing serious illnesses and their families. As part of a great interdisciplinary team, this individual helps patients cope with the emotional and psychological challenges that come with serious illness, such as anxiety, depression, and grief. They provide counseling, support, and resources to help patients and their families manage these challenges and improve their overall well-being. At the same time they are also an advocate helping patients define their own health goals and find ways to manage health challenges on their own terms.

The Supportive Care SW will provide support and counseling to patients' families, who often experience significant emotional and practical challenges when a loved one is seriously ill. They will help families navigate difficult decisions, cope with grief and loss, and connect with resources in their communities.

You will work remote in anywhere in the continental United States, and you are willing to work on California hours from approximately 9:00am to 5:00pm Pacific Time.

You will focus on:
  • Providing primarily telephonic post discharge care coordination and management services for members with an emphasis on Altais' full risk Medicare population.
  • Activities include clinical assessment, member/family/caregiver education, development, and implementation of individualized care plans.
  • Utilizing advanced clinical judgement and critical thinking skills to perform care management duties.
  • Working collaboratively with inpatient care managers, Referral Services, Population Health, and other internal teams.
  • Engaging with BTP primary care providers, and specialists, as well as hospital, skilled nursing, and health plan partners.
  • Participating in health plan and quality audits as needed.

The Skills, Experience & Education You Bring
  • MSW
  • 2+ years of palliative care
  • Epic Experience is preferred
  • 3 years experience in a clinical/health care environment with demonstrated experience at a professional level in quality improvement/clinical program oversight/project management.

Final determination of a successful candidate's starting pay will vary based on several factors, including, but not limited to education and experience within the job or the industry. The pay scale listed for this position is generally for candidates that meet the specified qualifications and requirements listed on this job description. Additional pay may be determined for those candidates that exceed these specified qualifications and requirements. Physical Requirements: Office Environment - roles involving part to full time schedule in Office Environment. Based in our physical offices and work from home office/deskwork - Activity level: Sedentary, frequency most of workday.
