Then we have an exciting role for you!Dartmouth Health Manchester is currently seeking an experienced Family Medicine APRN or PA to work with our Family Medicine team to manage the health and wellness of our patients. As part of a Medical Home, you will work collaboratively to provide comprehensive care to our patients in Southern New Hampshire. Working under the general direction of the medical director, you will practice autonomously as well as in collaboration with physicians, APRNs, and PA colleagues and other healthcare providers to provide primary healthcare to patients in our practice.Required Qualifications:
- 3+ years working in a primary care clinical setting
- Prior experience managing a primary care panel of patients
- Collegial and inclusive environment and referrals
- Embedded behavioral health specialists and LICSWs
- Excellent care coordination
- Nurses available for triage
- Ties to the Academic Medical Center

Minimum Qualifications:
- Graduate of an accredited APRN or PA program.
Required Licensure/Certification Skills:
- Eligible for APRN or PAlicensure in the State of New Hampshire.
- Eligible for Federal DEA Registration.
