Stroke Outpatient Clinic APP

Provides neurologic specialized healthcare to individuals and families suffering from stroke and other cerebrovascular disease in the ambulatory setting. Assesses and manages the healthcare needs and problems within neurology, emphasizing health promotion and disease prevention as well as the diagnosis and treatment of acute and chronic illness. Functions as part of the provider team to provide care to the departments panel of patients and, may have a defined panel of patients who identify the APP as their primary stroke specialist. Additional opportunities may include aspects of teaching, research/QI, and stroke program growth and development.Under the supervision of the Neurology Chair, Lead APP, and Cerebrovascular Disease Section Chief, the associate provider will coordinate services throughout the Neurology Department to drive the highest quality, efficiency, and community impact for cerebrovascular care. Primary clinic location is at the main campus, academic medical center building.Key Clinical Responsibilities:
  • Assesses physical and psychosocial health status of patient and family by taking health histories and conducting comprehensive physical examinations.
  • Orders appropriate diagnostic/screening tests to evaluate patients current health status.
  • Formulates diagnosis and implements treatment plan to manage acute illness and stable chronic illness consulting with and making a referral to physician colleagues and other health care providers as indicated.
  • Consults and collaborates with a department physician or appropriate specialist to plan and manage the care for patients with unstable or complex acute or chronic illness. Facilitates transfer of patient to the primary care physician when the complexity of care exceeds scope of practice.
  • Evaluates patients progress during follow up visits and implement revised plan of care.
  • Prescribes and/or dispenses medication.
  • Provides patient education with emphasis on the promotion of health and wellness and the prevention of illness and disability.
  • Provides guidance and counseling for both individuals and families.
  • Demonstrates patient advocacy that ensures the patients right to privacy, confidentiality, and informed consent.
  • Serves as a mentor for new APPs, APP residents, and students.
  • Assumes responsibility to renew licensure in a timely manner and maintain DEA registration.
  • Attends continuing education programs to ensure ongoing professional growth and development to meet licensure requirements.
  • Performs other duties as required or assigned.

Requirements:
  • Graduate of an accredited APRN or PA academic program
  • National specialty certification as FNP, WHNP, PNP, ANP or certification through the National Commission on Certification of Physician Assistants (NCCPA)
  • Eligible for RN licensure in the State of NH (APRN only)
  • Eligible for APRN or PA licensure in the States of NH and VT
  • Eligible for Federal DEA Registration
  • BLS certification
