Our client, a community-based healthcare center providing culturally competent, community-based care and addressing the special healthcare needs of underserved, low-income New Yorkers located in New York City, is hiring an Ambulatory Primary Care Nurse Practitioner. Join a team dedicated to providing programs and services that include primary and preventative care, gynecological care, reproductive health services, cardiology, podiatry, psychiatry, audiology, ophthalmology, physician house calls and physical/occupational therapy as well as comprehensive HIV care. They are an FQHC (National Health Service Corps) and all of their providers are entitled to the tuition waiver.

Responsibilities
- Provide evidence-based Primary Care to our patients
- Be an integral of the collaborative care team which includes nursing staff and other services such as Health Education, Nutrition, Social Work and Behavioral Health.
- Work with a dynamic team providing care to medically underserved communities throughout New York City.
- Graduation from a professional degree program as a Nurse Practitioner
- Current license as a APN in NYS
- DEA license
- Valid Family Nurse Practitioner N.Y.S. License to deliver medical care
- post-graduate/residency experience delivering care in a Primary Care/Ambulatory Care setting.
