Jefferson Health and Thomas Jefferson University are seeking an experienced Geriatric Medicine leader to develop, expand, and innovate geriatric clinical programs across our health system. This role will involve creating and executing a visionary strategy for clinical geriatrics throughout Jefferson Health.
Position Overview:
The Enterprise Clinical Lead for Geriatrics will join the faculty of the Division of Geriatric Medicine within the Department of Family and Community Medicine at Sidney Kimmel Medical College. The successful candidate will provide clinical leadership across Jefferson Healths regions to support and enhance geriatric services at an enterprise level. Key responsibilities will include:
- Identifying priorities for geriatric care and leading the implementation of various initiatives, such as ambulatory care, hospital consultation, home-based primary care, post-acute care, and population health programs.
- Collaborating closely with colleagues across Jefferson, including those in the Division of Geriatric Medicine, Jefferson Primary Care, and other clinical and operational leaders across Jefferson Healths three regions. These regions together encompass 17 hospitals and over 150 ambulatory sites.
- Partnering with the Office of Population Health and Jefferson Health Plan to align initiatives with Jefferson's broader healthcare goals.
- Maintaining an active clinical practice in geriatrics.
The candidate will report to the Director of the Division of Geriatric Medicine and will be supported by the Associate Administrator for the Division of Geriatric Medicine. The appointment will include a faculty position at Sidney Kimmel Medical College, with rank commensurate with experience and qualifications.
Qualifications:
- MD or DO degree
- Board Certification in Geriatric Medicine or Certificate of Added Qualification (CAQ) in Geriatric Medicine
- Primary board certification in either Internal Medicine or Family Medicine
- Minimum of 5 years of experience in leadership and development of clinical geriatric programs, including operational management, quality improvement, and enhancing patient experience
- Proven ability to build and lead interprofessional teams
- Ability to effectively collaborate in a complex, matrixed environment
- Strong understanding of geriatric clinical models, including design, business modeling, productivity, quality, patient experience, and equity in both traditional and value-based care settings
- Experience with community collaborations for clinical programming is preferred
- Demonstrated commitment to health equity
