Job Description

A Brooklyn nonprofit child services agency is looking to add a new, Licensed Practical Nurse to join their growing team on a full-time basis at one of their Article 29i Clinics as the Nurse Supervisor.

REQUIREMENTS:

  • Current Licensed Practical Nurse in New York State
  • CPR certification and completion of required courses in Child Abuse and Infection Control. .
  • 1 year of experience treating children/adolescents in the nursing field.


Position Overview: Reporting directly to the Nursing Supervisor, the LPN will be tasked with coordinating and delivering top-quality care to clients.


Core Responsibilities:
Care Coordination

  • Work with the Medicaid Managed Care Liaison to establish a medical home that meets the youth's current and future health needs by identifying Medicaid-approved providers.
  • Coordinate with Medical Assistants to schedule care (primary, dental, mental health, specialty) and obtain medical records.
  • Act as a bridge between multiple providers in delivering health services to children in care.
  • Record client care by documenting in patient and department records, ensuring ongoing compliance through audits.
  • Share relevant health information with clients, the court, and multidisciplinary team members as appropriate.
  • Attend case conferences as necessary.
  • Support caregivers with the medical, dental, and mental health needs of children in their care.
  • Ensure compliance with medication rules for children prescribed psychotropic or other medications.
  • Communicate with hospitals and attend necessary meetings when a child is hospitalized.

Client Care

  • Conduct routine screenings for substance abuse, developmental health, mental health, etc., as directed.
  • Perform physical checks for signs of abuse.
  • Provide psychoeducation on chronic conditions (Asthma, Diabetes), health education on sexual and reproductive health.
  • Carry out medical orders, such as tests, tasks, and hospitalizations as prescribed by a physician.
  • Record vital signs, height, weight, and other observations.
  • Administer immunizations and screenings for hearing and vision.
  • Offer first aid and emergency care as needed.
  • Collect lab specimens.

Additional Duties

  • Foster a supportive environment by providing emotional and psychological support to clients and families.
  • Secure any required consents.
  • Complete other assigned duties.
  • Maintain professional boundaries with clients and staff.
  • Keep a safe and secure medical environment.
  • Receive medication deliveries, verify accuracy, and store medications safely.
