Job Description

Job Title: Licensed Practical Nurse

Job Summary:

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our Intensive Mobile Treatment (IMT) team. As an LPN on our team, you will provide holistic care to justice-involved individuals with complex mental health needs, promoting physical and mental health integration and wellness. Your expertise will be essential in conducting health screenings, administering medications, and connecting clients with primary and specialty care services. Collaborating with a multidisciplinary team, you will work to improve client outcomes and promote recovery, rehabilitation, and community integration. Salary: The salary for this role is $70,000 per year.

Shift Hours: This position is full-time Monday through Friday 9:00 AM - 5:00 PM

Location Address: 2090 Adam Clayton Powell Jr Boulevard, New York, NY 10027. Our office is easily accessible by public transportation.

Workplace Flexibility: Flexible - Staff have flexibility to complete their work day from home or another suitable space, typically after reporting to the office and/or engaging in field or community-based work.

What you will be doing:

  • Improve the overall health of program participants, including promotion of physical and mental health integration.
  • Conduct health screenings, provide linkage to primary and specialty care, and educate on health and wellness.
  • Perform blood draws, monitor and administer psychotropic injections consistent with consumer needs preferences and practice standards, and prepare of medication to be dispensed.
  • Complete progress notes and assessments in the electronic health record within a timely manner.
  • Work with external stakeholders – such as hospitals, shelters, jails, primary care – to achieve positive client outcomes.
  • Provide services in a strengths-based, recovery-oriented, trauma-informed, and culturally sensitive manner.

What we are looking for:

  • Active New York state license as a Licensed Practical Nurse;
  • At least two years of experience working with adults who have needs in the areas of mental health, substance use, trauma, violence risk, homelessness, frequent psychiatric hospitalizations, and criminal-justice involvement;
  • Previous use of an electronic health record, Footholds AWARDS preferred;
  • Strong commitment to social justice and the CASES mission;
  • Individuals with lived experience are encouraged to apply.

Preferred but not required:

  • Experience in a multidisciplinary setting and field-based clinical work preferred;
  • Fluency in Spanish.
