*** Our Home Care Agency supports geriatric patients living in Gaston, Lincoln, Cleveland, and Rutherford counties. ***

Job Summary: The home care agency supervisor is responsible for the care the CNA staff provides and for directing the office staff. They are responsible for ensuring participants' care is coordinated and managed appropriately according to state and federal guidelines. The home care agency supervisor plans, implements, and evaluates the patient's care plan for appropriateness to the individual patient's needs. Directing and developing staff;

Education: RN Degree

Experience: 1-2 years working as an RN Supervisor, with at least one year's experience working with the frail elderly population. Home Care experience preferred

Licensure, Registry, or Certification Required: Currently listed as RN in North Carolina. They must have no substantiated findings of neglect, abuse, or misappropriation of patients. A valid driving license with personal vehicle insurance is required. A dependable means of transportation is mandatory.

Special Training: Good driving record; able to communicate effectively both verbally and in written form; knowledge of and care for adults with chronic health conditions. Only act within the scope of their authority to practice. The home care agency supervisor plans, implements, and evaluates the patient's care plan for appropriateness to the individual patient's needs.

Immunizations: Be medically cleared for infectious diseases and have all immunizations up-to-date before engaging in direct participant contact


Key Responsibilities:

• Accomplishes nursing human resource objectives by selecting, orienting, training, assigning, scheduling, coaching, counseling, and disciplining employees; communicating job expectations; planning, monitoring, and appraising job contributions; recommending compensation actions; adhering to policies and procedures.

• Meets nursing operational standards by contributing information to strategic plans and reviews; implementing production, productivity, quality, and customer-service standards; resolving problems; identifying system improvements with the Agency Director

• Meets nursing financial standards by providing annual budget information; monitoring expenditures; identifying variances; implementing corrective actions with direction from the Agency Director

• Maintains home care guidelines by writing and updating policies and procedures.

• Assures quality of care by developing and interpreting the nursing division's philosophies and standards of care; enforcing adherence to state regulations

• Provides patient and health care team information by answering questions and requests.

• Resolves patient needs by utilizing home care coordinators and team strategies.

• Maintains a safe and clean working environment by designing and implementing procedures, rules, and regulations, calling for assistance from other health care professionals.

• Maintains documentation of patient care services by auditing participants and department records.

• Maintains professional and technical knowledge by attending educational workshops and reviewing professional publications;

• Maintains a cooperative relationship among healthcare teams by communicating information, responding to requests, building rapport, and participating in team problem-solving methods.

• Accomplishes organization goals by accepting ownership for accomplishing new and different requests and exploring opportunities to add value to job accomplishments.

• Completes all initial assessments, re-assessments, and supervisory visits every 90 days. Is on call evenings and after hours and available when staff is in the homes providing care.
