Lāna'i Community Health Center (LCHC), located on the island of Lāna'i, is deeply rooted in a unique history of transitions and cultural diversity. Role Overview Within the scope of licensing as an RN and APRN-Rx, the Nurse Practitioner (NP) will provide general medical care, assessments, evaluation, treatment and some minor surgery for patients under the scope of their licensure. In collaboration with the Associate and Medical Director, NP shall contribute to the clinic's daily operations and management of staff, as delegated by their licensure within the State of Hawaii. LCHC is a patient care, teaching and research organization and provides workshops, education sessions and outreach - all employees should expect to be involved with these activities the extent to which will depend upon their position in the organization.
Requirements
Provides and renders medical care to patients including examination, diagnosis, treatment and referral to appropriate health providers or agencies as required. All medical care will be provided under the established policies, procedures, and protocols of Lanai Community Health Center.
Maintains accurate and thorough medical records through chart documentation. Completes medical forms as required by applicable policies and procedures. Further, utilizes the electronic medical record in a meaningful use manner.
Ensures compliance with Lana`i Community Health Center policies and procedures, including those outlined in the Lana`i Community Health Center Employee Handbook, along with any supplemental policies and procedures.
Maintains knowledge and complies with all occupational safety guidelines. Ensures all reasonable precautions are performed to prevent accidental injury on the job.
Participates in staff meetings and conferences, along with attending all training sessions as assigned.
Practices and espouses the LCHC patient care philosophy: All-inclusive culturally sensitive Patient Centered Medical Home care integrated with behavioral health, outreach, and case management.
Carries out other related duties as required and assigned by the Associate Medical Director, Medical Director, and/or Executive Director.
Participates actively in education and training programs for students rotating through LCHC.
Participates in outreach, school, and community education and workshop presentations.
Participates actively in research projects, including providing patient care in accordance with research project protocol, promoting participation, data collection, analysis, reporting, writing, and the like.
Qualifications
Training and certification required for RN (bachelor's level), and graduate level Advance Practice Nurse, (MSN, DNP). Certification and unencumbered licensure as a Nurse Practitioner (APRN-Rx), including applicable specialty certification as obtained.
Licensure as a Registered Nurse and an Advanced Practice Registered Nurse in the State of Hawaii
Minimum of 2 years experience as an RN in an outpatient setting.
