We currently have an exciting full time career opportunity available to join the HKNC team as a Medical Coordinator at our Destiny Home! Responsible for assisting the residence manager and the RN in the delivery and documentation of nursing services, in addition to carrying out traditional life coach activities for the residents.ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.Ensure that all medical appointments are scheduled in a timely manner.Accompany residents to medical appointments, recreation programs, inclusionary activities etc. Ensures an adequate amount of medication is always on site for each individual.Ensure that all medical appointments are up to date.Ensure that all medical documentation is reviewed and signed by the RN and/or Residence Manager. Obtain and maintain AMAP certification.File all medical documentation in the individuals’ medical binders.Reports all incidents to the Residence Manager and reports all medical issues/ injuries to the registered nurse and follows protocols for notification.Maintain the MAR and ensure the ordering of all medications, under the direction of the RN.Assist RN in maintenance of Individuals’ medical binders.Administer medication and monitor consumers’ vitals, including weight/blood pressure, as needed and report any variations to the RN for follow-up. Attending monthly staff meetings, annual and semiannual life plan meetings and in-service meetings as needed.Assist consumers with toileting as needed. Complete daily documentation in Nurse’s log of interventions.Review and monitor Bowel Movement charts daily and report any issues to RN and ManagerParticipate in pharmacy review, as needed, or requested by the RN.Monitor diets and menus as provided by the dietician.Order medical supplies as needed and discontinue medications when indicated by prescribing physician under RN supervision.Assist consumers in their personal routine and provide positive reinforcement as appropriate.Actively listens, builds trust and positive relationships, communicates effectively to direct consumers in their personal routine, ensures positive reinforcement as appropriateComplete residents service goals and collect data.Provide first aid treatment as needed under RN supervision.Maintain the order, safety, cleanliness of the residence.Interact with parents and professionals from outside Agencies.To complete all other assignments as indicated by your supervisor and/or the direction of the RN.REQUIRED EDUCATION and/or EXPERIENCE High School Diploma or GED equivalentValid NYS Driver’s license required.At HKS we strive to provide a healthy work-life balance for our employees. we offer generous paid time off and comprehensive benefit package including:Pro-rated PTOPaid Holidays Fully Paid Medical Insurance*Voluntary Ancillary plans403b Program with Employer MatchTuition Assistance Career Advancement Opportunities *No-frills positions receive an additional 10% premium on the hourly rate of pay, but do not participate in the Union Health Insurance Plan.
