Position Summary
Under the supervision of the Patient Care Manager, the Float Nurse serves our patients by utilizing principles of palliative care and holistic philosophy to provide care that reflects skilled nursing judgement. The Float Nurse may work in different departments, facility, or home settings, adapting to varying patient needs.
Primary Responsibilities:
Admissions
- Responds as needed to referral sources ensuring all communication is prompt, professional and emphasizes a strong commitment to customer service
- Educates patients and their caregivers about Navian Hawaii's philosophy and services as well as the transfer process, insurance benefits, placement options and other necessary information.
- Obtains and communicates all relevant patient data for the determination of insurance benefits, discharge planning and care team follow-up.
- Facilitates a smooth transition from hospital and/or transfer to contracted facility and arranges for the timely acquisition of all necessary equipment and services.
- Documents and completes all activities in accordance with hospice protocol within the regulatory time requirement.
Nurse Case Management
- Responds to patient and family's hospice needs when assigned case manager is off-duty or as assigned by Patient Care Manager
- Provides case management services to best benefit patients and caregivers.
- Care provided reflects compliance with federal and state regulations, along with Navian Hawaii's policies and procedures.
- Documents all nursing and case management activities in accordance with hospice protocol and keeps attending physician informed of changes in patient condition.
- Provides assessment and reports to primary nurse of any changes in patient condition or treatment.
- Participates in Interdisciplinary Team conferences and shares expertise with the team. Requests support services from other Interdisciplinary Team members as appropriate.
- Demonstrates good judgment in seeking and accepting guidance; facilitates clear and honest communication with other team members.
- Gives end of day report to the RN on-call at the end of each workday.
- Travels as required for patient/family care.
General
- Supports Quality Improvement, Performance Improvement and Compliance initiatives in accordance with direction from the clinical leadership team.
- Engages in regular educational activities including those offered through Navian Hawaii, to improve own professional practice.
- Maintains anticipated productivity as set by the Patient Care Manager, Director of Clinical Operations, Medical Director, and President
- May be required to assume on-call duties after hours on holidays or weekends as assigned by the Patient Care Manager or designee.
- Other duties as assigned/required by the Patient Care Manager or Director of Clinical Operations
Knowledge and Skills Requirements:
- Ability to exercise sound, independent judgment in planning and providing care. Flexible and organized in planning.
- Skilled in providing holistic nursing care to patients and their families. Ability to identify and provide for the psychosocial needs of caregivers in concert with the other members of the Interdisciplinary Team.
- Good physical and emotional health.
- Ability to set priorities, research, problem-solve and make independent decisions with minimal supervision by using logic to identify key facts, explore alternatives, and propose quality solutions within the scope of the job.
- Must be able to multi-task with the ability to meet multiple deadlines and possess strong organizational, planning and follow-through skills.
- Excellent written and verbal communication skills as well as the ability to communicate clearly, concisely and effectively with all levels of individuals.
Experience and Educational Requirements:
- A Current Hawaii license, in good standing, as a registered nurse. BSN preferred.
- At least one year experience as a staff nurse in an acute hospital. Hospice and/or home health nursing experience preferred

Salary Range
$93,000 To $99,000 Per Year
