Social Worker Job at Springpoint Living in Denville

The Oaks at Denville is searching for an experienced Social Worker to join our team!

The Oaks at Denville is a Springpoint Senior Living community located in Denville, NJ. We are in search of a Part-Time Social Worker to assist the Social Services department, and to provide medically related social services in order to attain & maintain the highest practicable physical, mental, and psychosocial wellbeing of each resident.

Responsibilities:

• Assess the social/emotional needs of each resident within an appropriate time frame.
• Perform documentation duties as required.
• Interview residents/families as necessary in a confidential, private setting.
• Involve the resident/family in planning social services programs.
• Refer residents/families to other social service programs/agencies where appropriate.
• Maintain a professional working relationship with medical representatives and other health related facilities and organizations.
• Participate in community planning related to the interests of the facility and the services and needs of the residents and families.
• Work with consultants as necessary and implement recommended changes as required or as appropriate which must be reviewed with the Director of Social Services prior to decision.
• Ensure the emotional, educational, social, and spiritual needs of the residents are met.
• Assist the Director of Social Services and/or the Admissions Coordinator with the adjustment period for new residents to the community.
• Work with the Resident Review Team to ensure that each resident's needs are met.

Education and Experience:

• BSW or MSW degree required.
• Must be a CSW, LSW, or LCSW in the state of NJ.
• Experience in Long Term Care setting preferred.

Salary Range $28-$31 / hour
