Job Description: This is an exciting opportunity to join the Intermountain Behavioral Health and Castell team to support Pediatric Collaborative Care, an innovative clinical model integrating proactive behavioral health services in the Pediatric Primary Care setting. Remote must live in Utah and be available to be on site periodically. This position will support various pediatric clinics in Utah. It is a full-time position that will allow you to work in a mostly remote/work-from-home capacity, with periodic travel to pediatric clinics from Utah County to Cache County. The purpose of the travel is for clinic caregivers to get to know you personally, and it will occur about once every 3-6 months for each clinic. Job Essentials Tracks and documents patient follow-up and clinical outcomes using a registry as a guide for daily work and patient engagement, monitoring patients (virtually) for changes in clinical symptoms, treatment side effects, or other complications. Provides brief therapeutic interventions to adolescents ages 11-17 using evidence-based techniques such as Behavioral Activation (BA) for adolescents, Cognitive Behavioral Therapy (CBT) for children and adolescents, Motivational Interviewing, and First Approach Skills Training. Performs screening, assessment, and treatment of various mental, emotional, and behavioral disorders, providing patient education around condition management and available treatment options. Reviews referrals and medical records to identify patients unable to adhere to a medical plan without additional assistance; enrolls patients willing to engage in care management services. Consults with the Pediatrician/APP and the Psychiatric Consultant to facilitate treatment plan changes for non-improving patients, inclusive of changes in medications, psychosocial treatments, or referrals for additional services. Supports and closely coordinates mental health care with the patient’s Pediatrician/APP, caregiver(s) and, when appropriate, other mental health providers. Acts as an advocate on behalf of patients and families, where appropriate, ensuring effective communication with the multidisciplinary care team, outside agencies, and other resources. Participates in regularly scheduled caseload consultations with the Psychiatric Consultant and facilitates resulting treatment recommendations to the patient’s Pediatrician/APP. Supports psychotropic medication management prescribed by Pediatrician/APPs, focusing on treatment adherence, side effects, complications, and the effectiveness of treatment. Works with the multi-disciplinary team to develop a discharge plan in order to provide further care, services, and referrals in order to ensure continuity of care. Coordinates with RN Care Managers as needed to address medical needs. Collaborates with multiple patient care teams and outside agencies to ensure that the patient receives quality care, a timely discharge, and to address other care delivery delays/issues. Minimum Qualifications Master's Degree in Social Work. Education must be obtained from an accredited institution. Degree will be verified. Current State Licensure as LCSW or LMFT. Computer Skills, i.e., email, word processing. Excellent written and verbal communication skills. Preferred Qualifications Experience with patients, families, and treatment teams. Pediatric experience Physical Requirements: Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies. Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations. Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc. Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling gurneys and portable equipment, including heavy items. Often required to navigate crowded and busy rooms (full of equipment, power cords on the floor, etc). May be expected to stand in a stationary position for an extended period of time.
