Title: RN/Disability Coordinator

Reports to: Health and Wellness Director (RN)

Position DescriptionSummary

Reports to the Wellness Director. Responsible to render medical care to the ill or injured according to health standing orders and/or physician orders. Observes patients, records significant conditions and reactions, and notifies the Wellness Manager or Physician of patients’ conditions and reactions to drugs, treatment, and significant incidents.

Essential Functions
  1. Takes temperatures, pulse, blood pressure, and other vital signs to determine the patient’s progress.
  2. May provide training to staff in areas of CPR, First Aid, and crisis intervention.
  3. Participates in medical examination of new students in accordance with the Department of Labor (DOL) guidelines.
  4. May screen patients and direct them to the Center Physician, Dentist, Health Services Coordinator, Counselor, or public health department as appropriate.
  5. Assists the Physician with patient examinations.
  6. Prepares patients for tests and examinations.
  7. Prepares and administers prescribed medications.
  8. Maintains medical equipment and inventory of supplies and medications.
  9. Maintains accurate medical records, updates records in a timely manner, and retrieves all outside patient medical visit records for continuation of care on center.
  10. Provides medical counseling/education to students.
  11. Models, mentors, and monitors the positive normative culture of the center.
  12. Acts as a responsible custodian for the assigned center property.
  13. Reports violations of ethical behavior.
  14. Suggests opportunities for continuous operational improvement and reduction of waste.
  15. Identifies and reports environmental health and safety concerns

Disability Coordinator Duties:

A DC must accurately enter the required data in the disability data collection and accommodation plan icons in CIS prior to or on the day of arrival for new students or soon after disclosure of disability whether the disclosure occurs via disability documentation, completion of a RA/RM/AAS Request Form, or verbally for existing students.

b. For students who disclose their disability prior to arrival and require TABE testing accommodations, this data must be entered prior to the administration of the first TABE test. If a student discloses a disability and requests testing accommodations after the administration of the first TABE test, then an accommodation plan must be entered into CIS as soon as possible after disclosure of disability (see R3.a) and must be entered prior to the next TABE test administration.

c. DCs will Be the only person with access to the disability data collection entry screen, disability data report and the accommodation plan report with notes report in CIS.

d. Generally, only the DCs will have access to the accommodation plan entry screen; however, if a designee is appointed to enter accommodation plans, this staff person can have access.

e. All center staff responsible for providing accommodations will have access to the accommodation plan (without notes) report in CIS.

f. Ensure Accommodation plans do not contain any medical or diagnostic information.

RequirementsRequired Education & Experience
  • Active, unrestricted license to practice in the center’s state
  • One year of health care experience, preferably in a health clinic
  • Experience working with youth preferred
Certifications/Competencies
  • Ability to assist the Physician with patient examinations
  • Ability to prepare patients for tests and examinations
  • Ability to prepare and administer prescribed medications
  • Maintenance of medical equipment and inventory of supplies and medications
  • Ability to promote the Career Success Standards (CSS) by modeling appropriate behaviors, mentoring students when necessary, and monitoring both positive and negative behaviors through interventions
  • Strong organizational skills
  • Excellent written and verbal communication skills
  • Computer proficiency required
Minimum Eligibility Qualifications
  • If position requires driving, a valid driver’s license in the state of employment with an acceptable driving record is required
  • I-9 documentation required to verify authorization to work in the United States
  • Ability to pass pre-employment drug test and background check
