Your role at BHSN:

As a Supported Housing Case Worker at BHSN, you will assist eligible candidates secure safe affordable housing. You will work closely with OMH Supported Housing clients, HUD Housing individuals, and Homesteads on Ampersand clients.

What your day might look like:

  • Screens and admits eligible candidates
  • Reviews residents’ needs and connects them with services that will help achieve success in independent living
  • Assists participants to develop individualized Housing Support Plans
  • Routinely meet with residents in their home environment
  • Complete inspections of units to ensure dwelling is safe and meets Housing Quality Standards (HQS)
  • Collaborate with treatment team including internal and external support systems within the community to identify and connect to needed resources/supports
  • Respond immediately to crisis situations and offer intervention support in conjunction with BHSN Crisis and/or treatment team providers
  • Provides ongoing support and referrals as needed
  • Work with private landlords in the community to help secure apartments
  • Act as a liaison between the tenant and the landlord to resolve housing issues quickly
  • Complies with BHSN and regulatory policies and procedures
  • Manages programs in Director’s absence
  • Other duties as assigned

Your skills and qualifications:

  • GED/HS Diploma required
  • Associates Degree in relevant Human Services with 2 yrs experience preferred
  • Good interpersonal skills
  • Excellent organizational skills
  • Valid NYS driver’s license
  • Computer savvy
  • Able to lift up to 20 lbs.; bending
