Job Title: Pharmacist Job Specialty: Pharmacy Job Duration: Full Time Shift: Days Guaranteed Hours: Not specified Experience: Not specified License: Unrestricted license as a registered pharmacist in any state Certifications: Basic Life Support for Healthcare Providers (CPR and AED, may be obtained within 1 year of hire) Must-Have: - Pharm.D. degree - Candidates licensed prior to 2003 with a Bachelor of Pharmacy degree will be considered with 3 years of experience in hospital and/or ambulatory clinic pharmacy - Completion of a nationally recognized (by ASHP) pharmacy residency program preferred Job Description: - Provide efficacious, safe, and cost-effective drug therapy, ensuring appropriate outcomes in the use of drugs and medicines - Prepare and distribute medications according to Federal and State laws, and provide patient counseling and education - Advise medical, nursing, and staff on drug therapy and maintain an appropriate drug inventory - Formulate and participate in quality improvement activities - Exercise independent professional judgment prior to filling and dispensing prescriptions, evaluating for appropriateness and safety - Consult prescribers for any issues and make recommendations for alternative therapies - Use independent judgment for patient counseling to ensure understanding and compliance with drug therapy - Conduct Drug Utilization Evaluation to improve patient care, educate medical staff, and achieve potential cost savings - Formulate policies and procedures affecting department operations - Review all prescriptions for appropriateness of therapy including legality, completeness, contraindications, interactions, allergies, adverse drug reactions, lab value abnormalities, and follow-up - Provide clinical consultation and clarification, resolving any concerns or questions before dispensing - Document all clinical activities and interventions accurately and completely - Verify finished prescription orders in a timely manner and maintain all required record keeping and documentation - Organize and prioritize work assignments, ensuring timely pharmacy services - Provide clear and accurate advice to patients regarding their medications - Perform chart reviews for prescription refills and medication therapy management - Write recommendations for providers and request prescription refills on behalf of patients - Provide drug information, drug therapy consultation, and staff education - Direct pharmacy support personnel and verify the activities of pharmacy technicians - Serve as a role model and educator for pharmacy students and technicians - Participate in clinical and administrative activities, quality control, and quality improvement within the department - Attend meetings and participate in committees as requested - Assure availability, preparation, and control of medications, selecting appropriate drugs for formulary - Engage in health promotion and disease prevention activities related to drug use and preventative drug therapy This position requires someone who can work independently, handle frequent interruptions, adapt to changes in workload, and respond quickly to emergency requests. Proficiency in reviewing medical history, labs, and medication orders using knowledge of pharmaceutical sciences is necessary, along with skills in communication, computer applications, and math for accurate drug dosage calculations.
