Job Title: Embedded BH Specialist - Medical Home Southwest Requisition Number: 37676 Employment Type: Full Time Division: BEHAVIORAL HEALTH BU Compensation Type: Salaried Job Category: Behavioral Health Hours Worked: Thurs-Sat 7A-7P Location: Medical Home Southwest Tarrant Shift Worked: Day Job Description: Description: The Embedded Behavioral Health Specialist is responsible for utilizing their subject matter expertise to provide mental health services, consultation, care coordination and other targeted interventions within the primary care setting to assist in meeting the behavioral health needs of patients. Typical Duties: Provides quick and accurate clinical assessments of mental health and substance abuse conditions. Provides brief, solution focused therapeutic engagement with patients as needed. Provides assistance in detecting and addressing behavioral health needs within the primary care setting. Coordinates and facilitates communication between primary care physician and psychiatrist when needed. Utilizes behavioral health expertise to promote change to improve behavioral and physical health conditions. Provides referral and community resource information. Assists with linkage to more intense psychiatric care as appropriate. Performs other related job duties as assigned. Qualifications: Required Education and Experience: Master's Degree in Social Work or Counseling. 2 plus year of clinical experience. 1 plus year of experience using Microsoft Professional Office Suite, to include Word, Excel, PowerPoint and Outlook. Required Licensure/Certification/Specialized Training: Licensure as an LMSW, LCSW, or LPC. Certification in Non-Aggressive Crisis Management; must be obtained while in orientation and maintained throughout employment. BCLS certification required but may be obtained subsequent to hire.
