Job Description:

The RN of the Office Based Addiction Program and the Chronic Pain Program is a member of the Primary Care Team who is responsible for operational assignments to ensure effective team-based care with the expectation of meeting the standards for patient Centered Medical Home.

Services include organizing, coordinating, and providing direct patient care, coordination and/or care management services and clinical medical support to providers for patients within the practice.

The RN– OBAT and Chronic Pain- is an integral part of the multidisciplinary care team responsible for ensuring that the primary care provider and practice team maintains a central role in the care process, by coordinating the care of these vulnerable patients, and ensuring the patients receive optimal care including acute illness management, chronic disease management, self-management coaching, and wellness/preventive care as they transition across multiple health settings and/or multiple physicians/providers.

Educational Requirements: Associate Degree in nursing required. A nurse with a BS or Master’s Degree preferred.

Experience Required:

Minimum two years’ experience in case management, disease management, home health care nursing, or intensive outpatient education and/or self-management support. Experience in practice coordination, ambulatory care, and/or staff supervision preferred. Demonstrated teamwork, collaboration with other staff, written/verbal communication skills. Experience with EMR; EPIC preferred.

Professional Licensure/Special Skills and Certification Required:

Licensed RN by MA State Board of Registration. BLS required; ACLS preferred. Certified managed care nurse or registered nurse case manager preferred.
