Position Summary
The Lifestyle Nurse Specialist drives integration of clinical and lifestyle care plans for management or reduction of disease and risk factors to support the organization and individuals to achieve health improvement goals and overall well-being.
The Specialist assesses health risks, navigates to resources and provides case management and behavior modification education, support, and accountability to assigned population with emphasis on lifestyle, behavior change and self-management.
The Lifestyle Nurse Specialist covers a geographic territory to deliver services to assigned populations and locations and is accountable for a designated patient caseload to facilitate the plan of care with the underlying objective of enhancing the quality of patient care, optimizing clinical outcomes and patient satisfaction while managing the cost of care.
The Specialist provides leadership in wellness and population based health improvements such as educational classes and programs for cohorts of patients, environment and culture changes to promote well-being. The Specialist also helps drive change through performance improvement.
Responsibilities
* Performs patient and population health assessments, stratification and navigation * Uses nursing skills to performs clinical and advanced assessments. * Assesses health/lifestyle behaviors, social determinants and readiness to change. * Uses nursing skills to accurately identify, stratify and prioritize individual, cohort and population care needs. * Uses motivational interviewing and behavior change models to prioritize patient goals and accountability methods. * Navigates and activates patients in appropriate resources/benefits. * Uses nursing judgment to provide case management and behavior modification of high-risk patients in assigned caseload.
Minimum Requirements
* Bachelor's Degree in Nursing from an accredited college or university required. * 2 years clinical experience in Lifestyle Medicine, Population Health, Disease Management, Case/Care Management, Motivational Interviewing, Behavior Change, Wellness Coaching, or Patient Education. * RN licensure in the State of NC or hold a Temporary License as a Registered Nurse in NC * 1 Advanced Certification required. * Evidence of a high level of skill in critical thinking, assessment, identification of problems, care planning, utilization, motivational interviewing and behavior modification.
