Wage Range: $142,480.00 - $166,504.00, Exempt

Position Overview:
The Family Nurse Practitioner is responsible as a provider of primary health care services who coordinates and refers clients for health care and health education.
What you will do:
* Provide comprehensive primary health care. * Coordinate patient care with other providers and agencies and advocate for client within health care systems. * Provide services in prenatal integrated health care of high-risk chemically dependent pregnant patients, in consultation and/or collaboration with obstetrical health care provider. * Perform initial and ongoing health risk assessments and counseling for high-risk lifestyle behaviors and/or familial diseases: ongoing drug and alcohol assessment and HIV screening. * Coordinate with contractual pharmacy, primary care providers, hospitals, and laboratories for required diagnostic procedures and delivery/births.
Qualifications
We would like to hear from people that have:
* Nurse Practitioner, unencumbered licensure with State of Oregon as Certified Nurse Practitioner and prescriptive authority. * Basic Life Support within introductory period. * Ability to pass a pre-employment or for cause drug tests. * Ability to pass criminal background and DMV checks.
