Positions Available - 2 FT LPN - 8a-430pm, M-F, holiday and weekend rotation

Summary of Position: The Licensed Practical Nurse is responsible for direct patient care.

Education and Experience:
  1. Graduation from an approved School of Nursing.
  2. Current LPN licensure in the State of Ohio.
  3. Minimum of two years of experience in Medical/Surgical, Oncology or community health nursing.
  4. Hospice experience preferred.
Special Skills and Requirements:
  1. Shall have a valid driver's license.
  2. Excellent customer service skills.
  3. Detail oriented with excellent communication, documentation, and organizational skills.
  4. Ability to follow instructions.
  5. Ability to work independently without supervision.
  6. Travel to other sites as needed. Travel may be in inclement weather.
  7. Ability to lift 25 lbs of supplies or equipment.
  8. Requires various amounts of sitting, standing, twisting, turning, bending, lifting and walking.
  1. Ability to assist with patient care activities such as ambulation, transfers, and positioning.
Responsibilities of Position:
  1. Provide competent nursing care to assigned patient through skilled symptom management, emotional support, appropriate teaching and timely response to patient and family needs including;
    1. Routine Home care Visits
      1. End of Life/Continuous Care
      2. Referrals
      3. Care Center Assistance
      4. Chart Reviews
      5. Other duties as assigned.
  2. Recognize and report promptly to the registered nurse any changes in the mental and/or physical status of the patient. Reporting should be written and oral when appropriate.
  3. Explain hospice services and benefits in regards to Medicare, Medicaid and Private Insurances and knows when to use Palliative Care Interventions' and ABN's.
  1. Obtain and enter orders from Medical Directors and Physicians when deemed appropriate.
  2. Order appropriate Durable Medical Equipment (DME) for patient.
  1. Facilitate continuity of care by coordinating admission of home-care patients to in-patient setting (hospital, nursing home), as needed.
  1. Work evenings and weekends as needed to meet the needs of the patients and families and agency backup schedule
  1. Complete all documentation as required by the organization in compliance with Agency policy, accreditation bodies, and governing requirements.
  1. Maintain clinical competency for assigned responsibilities and procedures.
  1. Other duties as assigned.
