RN – Care Center – PM - FT

Position Information: FT, Hourly, 36 hours per week, 6p-630am, weekend and holiday rotation

Summary of Position: The Primary Care Center Nurse is responsible for direct patient care in the Hospice Care Center.

Education and Experience:

  1. Graduation from an approved School of Nursing.
  2. Current RN licensure in the State of Ohio.
  3. Minimum of two years of experience in Medical/Surgical, Oncology or community health nursing.
  4. Hospice experience preferred.

Special Skills and Requirements:

  1. Shall have a valid driver’s license.
  2. Excellent customer service skills.
  3. Detail oriented with excellent communication, documentation, and organizational skills.
  4. Ability to follow instructions.
  5. Ability to work independently without supervision.
  1. Travel to other sites as needed. Travel may be in inclement weather.
  2. Ability to lift 25 lbs of supplies or equipment.
  3. Requires various amounts of sitting, standing, twisting, turning, bending, lifting and walking.
  4. Ability to assist with patient care activities such as ambulation, transfers, and positioning.

Responsibilities of Position:

  1. Provide competent nursing care to assigned patient through skilled symptom management, emotional support, appropriate teaching and timely response to patient and family needs including:
    1. Inpatient Care Center
    2. Routine Home care Visits
    3. SIA/Continuous Care
    4. Referrals and Admissions
    5. Chart Reviews
    6. Other duties as assigned.
  2. Provide patient/family services according to established Plan of Care and signed physician orders. Consult patient’s physician and/or Medical Director when necessary and initiate appropriate telephone orders for changes in Plan of Care.
  3. Coordinate IDG planning for assigned patients.
  4. Participate in the IDG analysis of the patient resulting in the development, implementation, and revision of the IDG Plan of Care.
  5. Teach, supervise, and evaluate the care provided by volunteers, hospice aides, and primary caregivers.
  6. Facilitate continuity of care by coordinating transfer/admission of home-care patients to in-patient setting.
  1. Facilitate continuity of care by coordinating transfer/admission of in-patient patients to home-care setting, if applicable.
  2. Explain hospice services and benefits in regards to Medicare, Medicaid and Private Insurances and knows when to use Palliative Care Interventions’ and ABN’s.
  3. Obtain and enter orders from Medical Directors and Physicians when deemed appropriate.
  4. Order appropriate Durable Medical Equipment (DME) for patient.
  5. Perform competent initial nursing assessments.
  6. Work evenings and weekends as needed to meet the needs of the patients and families and agency backup schedule.
  7. Complete all documentation as required by the organization in compliance with Agency policy, accreditation bodies, and governing requirements.
  8. Maintain clinical competency for assigned responsibilities and procedures.
  9. Other duties as assigned.
