Sanctuary Recovery Centers, a leading substance abuse treatment facility, is seeking a dedicated and certified Community Health Worker (CHW) to join our team. This full-time position involves providing comprehensive support to clients in our substance abuse treatment programs, including Intensive Outpatient Programs (IOP), residential treatment, and core services.

Key Responsibilities:

  • Client Engagement and Support:

    • Build trusting relationships with clients to support their recovery journey.
    • Conduct initial assessments and follow-up visits to monitor client progress.
    • Provide emotional support and guidance to clients and their families.
  • Care Coordination:

    • Assist clients in navigating healthcare and social service systems.
    • Coordinate appointments, follow-ups, and referrals to ensure seamless care.
    • Collaborate with healthcare providers, social workers, and other professionals to develop and implement individualized care plans.
  • Health Education and Outreach:

    • Conduct community outreach to raise awareness about substance abuse treatment options and resources.
    • Educate clients and their families about treatment plans, healthy lifestyle choices, and available resources.
    • Organize and facilitate health education workshops and support groups.
  • Advocacy and Resource Connection:

    • Advocate for clients' needs within healthcare systems and other social services.
    • Connect clients with community resources, such as housing, employment, and educational opportunities.
    • Work to remove barriers to care and support clients in achieving their recovery goals.
  • Data Collection and Reporting:

    • Collect and report health data to inform program planning and policy development.
    • Monitor and evaluate the impact of health interventions.
    • Maintain accurate and confidential client records.
  • Cultural Competence:

    • Deliver culturally sensitive and appropriate health education and information.
    • Serve as a bridge between clients and healthcare providers to overcome cultural and language barriers.

Qualifications:

  • Certification: Must be a certified Community Health Worker (CHW).
  • Education: High school diploma or equivalent required; additional training or certification in community health work preferred.
  • Experience: Previous experience in a healthcare or social service setting, particularly in substance abuse treatment, is highly desirable.
  • Skills: Strong communication, organizational, and interpersonal skills. Ability to work independently and as part of a team. Proficiency in data collection and reporting.
