Sanctuary Recovery Centers, a leading substance abuse treatment facility, is seeking a dedicated and certified Community Health Worker (CHW) to join our team. This full-time position involves providing comprehensive support to clients in our substance abuse treatment programs, including Intensive Outpatient Programs (IOP), residential treatment, and core services.
Key Responsibilities:

Client Engagement and Support:
- Build trusting relationships with clients to support their recovery journey.
- Conduct initial assessments and follow-up visits to monitor client progress.
- Provide emotional support and guidance to clients and their families.

Care Coordination:
- Assist clients in navigating healthcare and social service systems.
- Coordinate appointments, follow-ups, and referrals to ensure seamless care.
- Collaborate with healthcare providers, social workers, and other professionals to develop and implement individualized care plans.

Health Education and Outreach:
- Conduct community outreach to raise awareness about substance abuse treatment options and resources.
- Educate clients and their families about treatment plans, healthy lifestyle choices, and available resources.
- Organize and facilitate health education workshops and support groups.

Advocacy and Resource Connection:
- Advocate for clients' needs within healthcare systems and other social services.
- Connect clients with community resources, such as housing, employment, and educational opportunities.
- Work to remove barriers to care and support clients in achieving their recovery goals.

Data Collection and Reporting:
- Collect and report health data to inform program planning and policy development.
- Monitor and evaluate the impact of health interventions.
- Maintain accurate and confidential client records.

Cultural Competence:
- Deliver culturally sensitive and appropriate health education and information.
- Serve as a bridge between clients and healthcare providers to overcome cultural and language barriers.
Qualifications:
- Certification: Must be a certified Community Health Worker (CHW).
- Education: High school diploma or equivalent required; additional training or certification in community health work preferred.
- Experience: Previous experience in a healthcare or social service setting, particularly in substance abuse treatment, is highly desirable.
- Skills: Strong communication, organizational, and interpersonal skills. Ability to work independently and as part of a team. Proficiency in data collection and reporting.
