Position Summary:

Clinica Romero is seeking a compassionate and organized CHW-Patient Liaison to guide patients through the healthcare system. The ideal candidate for this role should have a deep understanding of the needs of the community, superb interpersonal skills, and a commitment to patient care. The ultimate goal is to enhance patient care by providing a personal connection and continuity of care throughout the patient's journey.

Duties and responsibilities:

• Assist patients in understanding Clinica Romero's health services.
• Assist patients with scheduling appointments and paperwork
• Referring patients to health care providers
• Coordinate appointments with nutritionists and other health professionals
• Assisting patients in understanding and completing necessary paperwork
• Educate patients about available resources and support services
• Serve as a liaison between patients and healthcare professionals
• Help patients with the enrollment and eligibility process
• Input patient demographic data in Clinica's system
• Refer patients to the enrollment and eligibility department
• Receive and make phones calls when needed.
• Provide emotional support and counseling to patients and their families
• Follow up with patients and monitor their appointments and eligibility process as requested
• Work with a multidisciplinary team to provide holistic care
• Participate in outreach and community engagement activities

Qualifications

• Excellent communication and interpersonal skills
• Ability to empathize and provide emotional support
• Strong organizational skills and ability to multitask
• A patient-oriented approach and excellent problem-solving skills
• Proficiency in EPIC, word and excel
• Experience working as a CHW and community organizing is a plus
