LPNs are important to our medical team by providing direct care to our patients. Your responsibilities will include collecting data to assist the care team in assessment of the patient. You will assist in the planning, implementation, and evaluation of their care plan after establishing with the care team each patient’s goals based on identified problems or needs. And when changes occur, work with the care team to modify their care plan. An LPN is a vital part of Finger Lakes Community Health – one of the fastest growing health centers in New York State. Additional qualifications: • High School Diploma or equivalent • A current unrestricted NYS LPN license and CPR certification required • Prior experience a plus • Outstanding organizational skills and attention to detail • Strong interpersonal skills and the ability to build relationships with personnel, patients, and community partners • Proven ability to handle confidential information with discretion • Excellent customer service and problem solving skills • The ability to work flexible hours -- some evenings are required • The ability to communicate fluently in Spanish and English preferred • A valid New York State driver’s license in compliance with FLCH drivers’ policy Department Nursing Employment Type Full-Time Compensation 20.75-30.08
