The Johns Hopkins Medicine Office of Population Health (OPH)
is a Johns Hopkins Health System department comprised of management and patient-facing clinical teams. Together, these teams support enterprise-wide efforts to improve quality and patient outcomes by implementing timely and site-appropriate care delivery and utilization management and to reduce health disparities by increasing patient access to services and advancing care integration. What you will do: The Care Manager is an integral member of the primary care team, tasked with leading efforts to engage patients and coordinating care for high risk and high need patients with complex health care and social needs. Working alongside the primary care provider, proactively anticipates and coordinates care services across multiple providers and care settings and provides comprehensive self-management and health education for targeted member populations with chronic disease conditions or high-cost specialty care. Expected to demonstrate knowledge of complex care management core competencies, disease prognosis and outcomes, and is responsible for ensuring member goals are achieved within a specified time period based upon individual needs, reasonable expectations and well-documented outcomes. Who Should Apply: Bachelor’s Degree from an accredited school of nursing
or
Bachelor’s Degree in social work. Current registered nurse or social worker with licensure in the state of Maryland. Minimum five (5) years clinical practice. Three (3) years care management. Salary Range: Minimum 31.45 per hour - Maximum 51.91 per hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.
