The Johns Hopkins Medicine Office of Population Health (OPH) is a Johns Hopkins Health System department comprised of management and patient-facing clinical teams.

With the goal of improving health outcomes for our patients and communities, we implement and optimize a strategic, innovative, and data-driven population health operating model across the continuum of care.

The Care Management Manager is responsible for operationalizing the daily Care Management workflow standards and processes, ensuring adherence to regulatory guidelines and the operations are consistent with JHHS and the Office of Population Health standards in the outpatient setting. The Manager will work with the Director ensuring adequate staffing and training to meet clinical and program needs. The Manager is responsible for measuring staff performance based upon JHHS policy and procedures and regulatory accreditation standards. Additionally, the Manager will assist with managing strategic relationships with client groups and community leaders.

Who Should Apply:

Bachelor’s Degree in Nursing required. Master's degree preferred.

Current registered nurse with licensure in the state of Maryland.

Certification in Case Management (CCM) preferred.

Minimum eight (8) years clinical experience, which includes a minimum of three (3) years in a managed care environment in case management role.

Minimum two (2) years management experience, preferably in a managed care environment.

Considerable knowledge of clinical protocols and practice management requirements in Case Management.

Experience with care coordination, discharge planning, and clinical documentation.

Salary Range: Minimum 44.31 per hour - Maximum 77.57 per hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.
