Detailed Description: This is a full-time, temporary, benefit-based position with an anticipated end date of October 31, 2027. The appointment is renewable at the discretion of the University and dependent upon performance and continued funding. The Case Manager will be responsible for overseeing clinical case management while maintaining a small clinical caseload. This role involves managing client cases from intake through treatment, coordinating care, and ensuring that clients receive the necessary support and resources. The ideal candidate will be a registered associate (AMFT, APCC, ACSW) with experience working with veterans or a strong interest in veteran-specific issues. Duties and Responsibilities: Clinical Case Management: Manage a caseload of clients, including veterans and their families, from intake through treatment. Develop and implement individualized treatment plans, coordinate care with other professionals, and monitor client progress. Coordination of Services: Facilitate communication between clients, their families, and other service providers to ensure a holistic approach to care. Assist clients in accessing additional resources and services as needed. Client Support: Provide direct clinical support to a small caseload of clients. Deliver evidence-based therapeutic interventions, conduct assessments, and address clients’ mental health needs. Documentation and Reporting: Maintain accurate and timely documentation of client interactions, treatment plans, and progress notes in accordance with clinic policies and regulatory requirements. Work closely with clinical staff and other departments to ensure the delivery of comprehensive and coordinated care. Participate in team meetings, case conferences, and other collaborative activities. Crisis Intervention: Provide support and intervention during clinical crises, including risk assessment and management. Develop and implement crisis management plans as needed. Professional Development: Engage in ongoing professional development and training to stay current with best practices in case management and telemental health. Persons offered employment in this position will be required to provide official education transcripts for degree verification purposes. Job Requirements: Minimum Qualifications: Master’s degree in Marriage and Family Therapy, Clinical Psychology, Social Work, or a related field. Current registration as an Associate Marriage and Family Therapist (AMFT), Associate Professional Clinical Counselor (APCC), or Associate Clinical Social Worker (ACSW) with the California Board of Behavioral Sciences (BBS). Minimum of 1-2 years of experience in a clinical setting, including direct case management responsibilities. Experience working with veterans is preferred. Certification or documented proficiency in one or more of the following EBP’s: PE, CBT, and CPT is required. Preferred Qualifications: Veteran-Specific Expertise Preferred: Apply knowledge and experience related to veteran-specific issues to provide tailored support and interventions. Stay informed about the unique challenges faced by veterans and their families. Performance Expectations- Knowledge, Skills and Abilities: Strong organizational and communication skills. Proficiency in telemental health platforms and a solid understanding of veteran-specific needs. Empathetic, detail-oriented, and committed to providing high-quality care. Ability to work effectively both independently and as part of a team. Knowledge in DSM-V-TR diagnoses. Knowledge of evidence-based practice, including targeted, evidence-based treatment modalities and measurement-based care. Experience in PTSD diagnosis and treatment.
