Job Summary

The Licensed Practical Nurse (LPN) is responsible for rendering care in assigned area in terms of individualized client needs, according to dependent and independent nursing functions, and conformance with recognized nursing techniques, procedures, and established standards based on the scope of nursing.

Essential Duties & Responsibilities

To perform this job successfully, an individual must be able to perform each essential duty and responsibility satisfactorily.
  • Utilize nursing knowledge and skills in the safe implementation of basic preventative therapeutic and rehabilitative nursing care of assigned patient/family as evidenced by documentation and observation of positive patient care outcomes.
  • Perform delegated independent nursing functions using established procedures, policies, guidelines and standards.
  • Administer medication accurately, observing patient response, as evidenced by documentation in the medical record and lack of negative outcomes.
  • Maintain and coordinate communication among patient/significant other, staff and physician as evidenced within medical record, client and health care team feedback.
  • Assist in assuring that effective communication exists with patient/significant other as reflected in the plan of care. Give punctual change of shift report consisting of pertinent and relevant information for assigned patients.
  • Work collaboratively with other members of the health care team in coordination of the patient's care as evidenced by other department feedback.
  • Use available written and AV materials specific to patient condition or disease. Utilize appropriate teaching methods such as one-on-one and group instruction, demonstration or discussion.
  • Assist patients, family members or other clients with concern and empathy; respect their confidentiality and privacy and communicate with them in a courteous and respectful manner.
  • Answer and refer telephone calls or other inquires to ensure accurate and timely communications are facilitated.
  • Other duties as assigned.

Physical Demands and Abilities

The physical demands and work environment characteristics described herein are representative of those that must be met by an employee to successfully perform the essential duties of this job. Reasonable accommodation will be made to enable individuals with disabilities to perform the essential functions.
  • Frequent lifting, carrying, pushing, pulling, stooping, bending, turning and stretching. Finger and hand dexterity required. Visual and hearing acuity essential.
  • Must frequently lift or move residents and objects weighing over 50 pounds.
  • Is subject to falls, burns from equipment, odors, etc. throughout the shift.
  • Is subject to exposure to infectious waste, diseases, conditions, etc.
  • Must be able to assist with the evacuation of residents.
  • Must be able to relate to and work with ill, disabled, elderly within the facility.
  • Frequently works in a well-lit or dimly-lit, tempered room.
Other Requirements
  • May be required to attend activities outside the facility to stay abreast of new developments, best practices, and statutory and regulatory changes.
  • May be required to work holidays
  • Maintain the confidentiality of all resident care information.
