Job Summary
Odyssey House Louisiana, Inc. is seeking a part-time non-exempt Licensed Practical Nurse. The schedule for the position is including holidays, nights, and weekends as needed for 24 hour Programs. The Licensed Practical Nurse (LPN) provides high-quality Client-focusedhealth care services that meet the assessed and individually planned needs ofall clients in OHL's Inpatient programs, Community Health Center and SoberingCenter. This position will work as part of themulti-disciplinary team (MDT) and is responsible for maintaining standards ofnursing care and competency, ethical treatment of patients, understanding ofaddiction and the medications used to treat substance use disorders, consistentdocumentation on care, review, and implementation of treatment orders.The Licensed Practical Nurse (LPN) assumes several keyresponsibilities in client care, staff training, and leadership.
Responsibilitiesand Duties
ProgramRequirements:
It is the duty and responsibility of the Licensed Practical Nurse to:
• coordinate/facilitatepatient care, work collaboratively and maintain communication with patient careteam (providers, counselors, etc.) to provide effective, timely, andappropriate patient management;
• observe,assess, and document clients experiencing S&S of withdrawals from drugsand/or alcohol on a daily basis;
• safelyadminister medications as ordered using the five rights and utilizing a MAR;
• conducta narcotic pill count at the beginning and end of each shift and management ofstock medicines;
• monitorspatient care by tracking patient charts for items such as complete admissionassessment, reconciliation of medications and continuity of nursing care;
• engageswith the individual's health care providers, as appropriate, to discuss ongoinghealth needs and interventions supportive of their fall prevention and healthimprovement plans.
• developsprevention and health improvement plan with the client and the participation ofthe individual's family or other support system that documents agreed-upongoals to improve health and safety to include: includes providing education onrisk and prevention, medication issues such as poly-pharmacy, and discussingexercise, nutrition, and social engagement.
• assistswith making necessary health care appointments and/or finding communityresources to increase strength, endurance, and activity levels.
• utilizesadvanced conflict resolution skills as necessary to ensure timely resolution ofissues;
• collaborateand communicate with multidisciplinary team at all phases of planning process,including initial patient assessment, planning, implementation, interdisciplinarycollaboration, and teaching and ongoing evaluation;
• coordinatethe management of patient's medication stock including pharmacy pick-up anddrop-off of meds, calling in refills for clients, and upkeep of how medicationis stored;
• conducturine drug screens on all residents as directed to do so;
• completeIncident Report form before the end of a shift if applicable; and
• maintainHIPAA guidelines; and
• performother duties as assigned.
Qualifications and Skills
Required:
Minimum qualificationsinclude the following:
• ValidLouisiana Licensed Practical Nurse License
• Highschool diploma or GED equivalent
• ElectronicHealth Record experience
• Excellentcommunication, organizational, and time management skills
Preferred:
Preferredqualifications include the following:
• Knowledgeof Narcotics Anonymous and Alcoholics Anonymous principles
• Knowledgeof ASAM criteria
• CurrentCPR certification
