Job Summary

Odyssey House Louisiana, Inc. is seeking full-time and Part-Time, non-exempt, day shift Licensed Practical Nurses. The schedule for the position is including holidays and weekends as needed for 24 hour Programs. The Licensed Practical Nurse (LPN) provides high-quality Client-focused health care services that meet the assessed and individually planned needs of all clients in OHL's Inpatient programs, Community Health Center and Sobering Center. This position will work as part of the multi-disciplinary team (MDT) and is responsible for maintaining standards of nursing care and competency, ethical treatment of patients, understanding of addiction and the medications used to treat substance use disorders, consistent documentation on care, review, and implementation of treatment orders. The Licensed Practical Nurse (LPN) assumes several key responsibilities in client care, staff training, and leadership.

Responsibilities and Duties

Program Requirements:

It is the duty and responsibility of the Licensed Practical Nurse to:

  • coordinate/facilitate patient care, work collaboratively and maintain communication with patient care team (providers, counselors, etc.) to provide effective, timely, and appropriate patient management;
  • observe, assess, and document clients experiencing S&S of withdrawals from drugs and/or alcohol on a daily basis;
  • safely administer medications as ordered using the five rights and utilizing a MAR;
  • conduct a narcotic pill count at the beginning and end of each shift and management of stock medicines;
  • monitors patient care by tracking patient charts for items such as complete admission assessment, reconciliation of medications and continuity of nursing care;
  • engages with the individual's health care providers, as appropriate, to discuss ongoing health needs and interventions supportive of their fall prevention and health improvement plans.
  • develops prevention and health improvement plan with the client and the participation of the individual's family or other support system that documents agreed-upon goals to improve health and safety to include: includes providing education on risk and prevention, medication issues such as poly-pharmacy, and discussing exercise, nutrition, and social engagement.
  • assists with making necessary health care appointments and/or finding community resources to increase strength, endurance, and activity levels.
  • utilizes advanced conflict resolution skills as necessary to ensure timely resolution of issues;
  • collaborate and communicate with multidisciplinary team at all phases of planning process, including initial patient assessment, planning, implementation, interdisciplinary collaboration, and teaching and ongoing evaluation;
  • coordinate the management of patient's medication stock including pharmacy pick-up and drop-off of meds, calling in refills for clients, and upkeep of how medication is stored;
  • conduct urine drug screens on all residents as directed to do so;
  • complete Incident Report form before the end of a shift if applicable; and
  • maintain HIPAA guidelines; and
  • perform other duties as assigned.
Qualifications and Skills

Required:

Minimum qualifications include the following:
  • Valid Louisiana Licensed Practical Nurse License
  • High school diploma or GED equivalent
  • Electronic Health Record experience
  • Excellent communication, organizational, and time management skills
Preferred:

Preferred qualifications include the following:
  • Knowledge of Narcotics Anonymous and Alcoholics Anonymous principles
  • Knowledge of ASAM criteria
  • Current CPR certification
