The Southeast Permanente Medical Group, (TSPMG)/ Kaiser Permanente is happy to announce an opportunity for a full-time Community Navigator to support internal medicine and family practice at our Jonesboro, GA medical office. This individual will be responsible for low acuity social /nonmedical needs to eliminate concrete barriers for members and families for the Georgia Region. Activities include planning and coordinating the social services and community resources necessary to respond to an individual member's health care needs to ensure continuity of care and the most efficient and effective use of resources. In addition, responsibilities include interacting with physicians, members/families, complex case managers, home care reviewers, inpatient care coordinators, medical office staff, and other Kaiser Permanente staff, member/family and others, and promotion of a high level of member satisfaction.
This position is in person Monday - Friday at our Jonesboro location and will support internal medicine and family practice. Ability to speak Vietnamese is plus.
The ideal candidate will have a high school diploma and at least two (2) years' experience in a health care setting.

Major Responsibilities/Essential Functions:
- Performs assessment for non-medical social service needs as requested by providers and departments.
- Assisting the LCSW and MSW with long term planning and identifying appropriate community resources.
- Communicates social services interventions to the member/family and makes changes as appropriate.
- Identifies, integrates, and coordinates Kaiser Permanente and community health care resources and services for members and works in conjunction with utilizing Thrive Local (Community Support Hub).
- Addressing SDOH -food insecurity, financial, transportation, utilities by accessing KP Resources and the Community that are impacting the quality of life with serious ill members.
- Using screening tools that exist in KPGA Health Connect and address the low acuity non-medical needs.
- Hybrid position requiring some travel to the different Medical Offices in the Kaiser Permanente Region to meet with patients and families as appropriate
- Knowledge of community resources required
- Working knowledge of all relevant federal, state, local and regulatory requirements including Medicare/HCFA
- Functional knowledge of computers
- Demonstrated customer service aptitude on a Customer Service Assessment
- BSW or MSW
- Experience with managed health care delivery including Medicare
- BLS training
