Kaiser Permanente/ The Southeast Permanente Medical Group Director, Continuum Clinical Operations The Director, Continuum Clinical Operations, is responsible for the operations, strategy, quality, and service of the clinical support and allied health teams within the Continuum, under the direction of the Executive Director of Clinical Operations and in partnership with physician leaders and Health Plan partners. The Director sets the strategy and clinical program priorities within Continuum Clinical Operations, in collaboration with physician leaders and other continuum partners. The Director also leads select projects for the GA region, process improvement efforts, clinical operations, and clinical team management. Clinical teams include the ECM Hub, Acute Care at Home (ACAH), Care Transitions, Geriatrics, and Palliative Care Programs, employing nursing, social work, chaplains, MAs, and other clinical and non-clinical staff. The Director ensures integration of clinical services across the continuum of care for KP members. Supervises and coordinates the activities of personnel providing geriatric care, palliative care, care transitions, ECM Hub and ACAH services. Responsible for clinical and business performance of the Continuum clinical support and allied health teams and assures that actions and goals in these areas are aligned with regional policy and direction. Proposes and implements short- and long-range plans for provision of existing and future services, including but not limited to palliative care, geriatrics, care transitions, ECM Hub, and ACAH departments. Works to provide a high-quality staff through recruitment, development of performance standards, training programs, competency evaluation, counseling, discipline, and termination in conformance with regional policies and procedures and in support of regional goals promoting diversity in the workplace. Identifies and investigates competency problems associated with daily operations and overall patient care within the programs and develops and administers corrective actions to ensure safe and efficient working conditions. Oversees staff productivity measures & monitors scheduling of personnel. Collaborates with physician leaders, the Quality department, and Health Plan leaders to develop, implement, and monitor strategic initiatives within the continuum, including the ECM Hub, Care at Home programs, Geriatrics, Palliative Care and Transitions of Care programs. Responsible for development & implementation of staffing and capital budgeting. Demonstrates sound resource stewardship. Educates and acts as a liaison between Continuum Clinical Operations and other KP departments, develops and oversees relationships with contracted hospice, home care and special services vendors, as well as with contracted hospitals to ensure effective patient care services. Develops & monitors quality indicators and intervenes as indicated; provides updates regarding performance outcomes to KP/TSPMG leadership. Advises department and physician leadership in a timely manner of activities both internal and external to the organization that may impact the continuum. Facilitates, in conjunction with continuum leadership and physician leadership, regular staff meetings in order to encourage education, effective communication and problem solving amongst staff. Acts as a resource to community primary care physicians, contracted consultants, Health Plan teams, provider relations and contracting staff in relation to continuum programs. Minimum Education Unrestricted current license as Registered Nurse, Board Certified Chaplain, or Licensed Clinical Social Worker in the state of Georgia. Preferred Education RN, BSN, MBA, MHA, Master of Divinity or master's in nursing or health care related field and/or APRN in adult, family, or geriatric nursing. Master of Business Administration, Health Administration, Public Health, or other related areas. Minimum Work Experience and Qualifications 5-7 years of healthcare experience. 3-5 years’ experience managing a palliative care, social work or case management department, or related clinical, allied health team. Proven track record of achieving objectives through motivating and leading teams. Ability to establish positive, professional relationships with senior leadership and management team members. Facilitation, conflict resolution and negotiating skills. Proficient with the use of Microsoft applications. Demonstrated organizational and problem-solving skills, as well as strategic and systems thinking capability. Preferred Work Experience and Qualifications One year of experience supervising an interdisciplinary team. Experience in Performance Improvement methodologies (i.e., Six Sigma Green Belt or higher). Required Licensure, Certification, Registration or Designation Unrestricted current license as Registered Nurse or Licensed Clinical Social Worker in the state of Georgia or Board-Certified Chaplain.
