We are currently looking for qualified candidates for our Promotores/Community Health Worker Supervisor role. This position reports to the Wellness Director and is a Regular, Full Time, Exempt role with a starting annual salary range of $75,400 to $83,000 with full benefits.
Primary Objective:The Bilingual Promotores/Community Health Worker Supervisor at Mission Neighborhood Health Center (MNHC) is responsible for leading and supervising a team of Promotores/Community Health Workers (CHWs) who provide health education, outreach, and access to services for the diverse communities in the Mission and Excelsior Districts. The Supervisor will guide the team in promoting MNHC’s comprehensive health services, including primary care, pediatrics, HIV prevention, food and housing support, homeless services, with a focus on serving the Latino, Spanish-speaking population. This role will ensure that MNHC’s outreach efforts align with the organization’s mission and strategic plans and priorities of advancing health equity and delivering high-quality care to the community.
Essential Functions/Responsibilities
Team Supervision & Leadership
- Lead, supervise, and mentor a team of Promotores/Community Health Workers, ensuring effective outreach and engagement with the Mission and Excelsior communities.
- Schedule and conduct regular team meetings to discuss outreach strategies, review progress, and address any challenges.
- Oversee the training and development of CHWs/Promotores, providing guidance to improve community outreach and health education efforts.
- Monitor and ensure accurate documentation of outreach activities, community engagement, and service referrals.
- Collaborate with other MNHC departments to integrate Promotores/CHWs into broader health service initiatives.
- Lead outreach initiatives that promote MNHC services, such as primary care, family planning, OB/GYN, HIV prevention, and homeless services, especially to Spanish-speaking, Latino communities.
- Develop culturally relevant and language-appropriate health education materials to be used by Promotores/CHWs in outreach activities.
- Build relationships with community members, local organizations, and healthcare providers to enhance MNHC’s community impact and improve access to care.
- Act as a liaison between community members and MNHC services, ensuring seamless referrals and continuity of care.
- Work with the Director of Population Health and Quality Improvement to design and implement health education programs addressing community needs, including chronic disease prevention, maternal and child health, women’s health, food and housing, mental health and wellness programs.
- Evaluate the effectiveness of outreach and educational activities, making data-driven recommendations for program improvements.
- Ensure that program activities comply with grant requirements and align with MNHC’s mission and goals. Works with other community partners and oversees contract relationships.
- Assist in preparing reports and presentations on outreach outcomes for internal and external stakeholders.
- Advocate for the health needs of the Latino, Spanish-speaking community, addressing barriers to care such as language access and transportation.
- Train CHWs/Promotores to serve as advocates for clients, empowering them to navigate the healthcare system effectively.
- Foster leadership opportunities for CHWs/Promotores to represent MNHC at community events, health fairs, and local forums.
Qualifications
Required:
- Bilingual (English/Spanish) – Strong verbal and written communication skills in both languages.
- Minimum of 3 years of experience as a Community Health Worker, Promotor, or similar role, with a focus on serving Latino communities.
- Minimum of 1 year of supervisory or leadership experience.
- In-depth understanding of the cultural, social, and healthcare needs of Latino, Spanish-speaking populations in San Francisco.
- Experience in community outreach, health education, and program coordination.
- Proficiency or willing to learn Microsoft Office Suite and electronic health record (EHR) systems.
- Excellent organizational and leadership skills.
- Experience working in a nonprofit organization or community clinic serving the Latinx community.
- Ability to travel between MNHC locations in the Mission and Excelsior Districts.
- Occasional evening and weekend work may be required for community events.
