MGH Back Bay is a med-peds practice in the beautiful Back Bay neighborhood of Boston. We provide comprehensive Primary Care healthcare for patients and families of all ages.

The Ambulatory Primary Care Staff Nurse at MGH Back Bay is a Registered Nurse who is responsible for assuring competent, compassionate, individualized nursing care for patients and families, including delegation to and supervision of non-professional clinical and support staff. She/he collaborates with all members of the care team to ensure that patient care is coordinated, comprehensive, and that care transitions are seamless. She/he collaborates and actively engages with the care team and practice leadership to identify opportunities and solutions to improve quality, patient access, and cost savings.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

  1. Provides competent, compassionate patient-and-family-centered evidence-based nursing care utilizing the nursing process and interdisciplinary teamwork, and collaboration.
  2. Utilizing their assessment, critical reasoning and astute clinical judgment, the nurse expedites appropriate care and treatment of the patient.
  3. Integrates knowledge of each patient’s history and unique circumstances with their knowledge of chronic disease management to provide longitudinal care to a panel of patients.
  4. Documents, in a timely manner, nursing assessment, interventions, plan and outcome in the patient record.
  5. Creates a therapeutic relationship with patient and family.
  6. Utilizing a variety of telecommunications technologies, the nurse assesses, triages, provides nursing consultation, and performs follow-up and surveillance of patient status and outcomes.
  7. Provides health education to facilitate patient treatment plan.
  8. Actively empowers patient and family to effectively manage their health care and navigate the health care system.
  9. Respects and supports patient and family’s cultural beliefs and values, individual needs, health goals and treatment preferences.
  1. Collaborates with all members of the health care team to ensure coordination of care.
  2. Reviews and follow-ups with patients, and members of the health care team, that patients are current with all necessary tests, screenings and follow-up care.
  3. Tracks and follows-up with care team and patient on all critical referrals and test results.
  4. In collaboration with the health care team, evaluates effectiveness of care and interventions for assigned group of patients.
  5. Communicates with members of the health care team−in-patient and/or in the community− to ensure continuity in care transitions.
  6. Actively engaged in creating a healthy, respectful work environment.

  2. Identifies opportunities, and solutions, to ensure a safe, efficient and cost effective practice environment.
  3. Demonstrates compliance with regulatory requirements.
  4. Participates in quality and safety initiatives.
  5. Generates innovative ideas to improve care delivery and work flow in the practice environment.
  6. Precepts and mentors new members of the team.
  7. Participates in departmental and organizational committees and initiatives.
  8. Interprets hospital and departmental policies and procedures to nurses and other members of the health care team.

SKILLS/ABILITIES/COMPETENCIES REQUIRED:

  • Computer literacy in Word, Power Point, and Excel preferred.
  • BLS Certification.
  • Proficiency in wound care, medication and vaccine administration; respiratory treatments; use of emergency equipment such as AED.
  • Excellent written and verbal communication skills.
  • Effective interpersonal and communications skills.
  • Effective counseling skills: both individual and group.

WORKING CONDITIONS:

Patient care in an out-patient, academic, primary care setting.

LICENSES, CERTIFICATIONS, and/or REGISTRATIONS (if applicable):

Current licensure as a Registered Nurse in the Commonwealth of Massachusetts.

EDUCATION:

Graduate of an accredited nursing program. Bachelor of Science in Nursing preferred; new graduates must have a BSN degree.

EXPERIENCE:

Previous experience in Ambulatory or Primary Care, with Medical Homes, and the impact of Accountable Care Organizations and the changing health care climate preferred.
