Job Description
An employee in this class is responsible for providing proactive and preventive care through an integrated team approach with medical providers, case managers, patients, other stakeholders, and community support systems. Work involves follow-up with frequent EMS patients to schedule appointments and arrange programs of preventive care to prevent or reduce emergency needs. The employee also performs assessment and prior approval for Medicare patients for repetitive transport and responds to calls as a paramedic in a Quick Response Vehicle.
Examples of Job Duties
Provides proactive care intended to help improve the overall health and wellness of program participates; follows up with repeat patients to expand access to non-urgent health care through in-home patient visits and/or community based services; performs assessment of patients; contacts physicians regarding patient encounters; assists patients with contacting community resources to assist with needs such as meals, prescription assistance and with navigation to appropriate destinations.
Works with various community partners to prevent overdoses; performs rehabilitation consultations and referrals; runs OD and cardiac arrest calls to back up EMS crews as needed; works with peer support specialists to improve outcomes.
Performs assessment and prior approval for Medicare patients for repetitive transport; tracks renewal calendar; compiles and sends in medical records; obtains physician signature for medical necessity.
Works in cooperation with various stakeholders and key medical providers to offer coordinated and integrated care, especially for patients with chronic diseases and substance abuse disorders; works with stakeholders and EMS agency to provide community education to prevent injury and illness; works with patients to reduce utilization; maintains clear and concise communications in compliance with HIPAA standards.
Visits patients in their homes to ensure proper medication use; following through on doctor appointments, etc; serves as "navigator" of community resources connecting patients to agencies and services they may not be aware of and helping reduce barriers that prevent access to care; coordinates with resources such as VA, home health, hospice, senior center, mental health agency, etc.
Performs critical stress debriefings with stress dogs for law enforcement, fire and EMS staff.
Prepares a variety of records and reports on activities and patient information; tracks patient calls for service; documents visits and maintains other necessary data; ensures all necessary reports and records and prepared and maintained.
Attends classes and seminars to stay abreast of changing regulations, consults with state personnel and assures that County's practices and procedures remain complaint with state regulations.
Responds to major incidents as a paramedic; responds as paramedic during times of staffing shortages or during any major incident to assist in the emergency.

Qualifications
Graduation from college with an associate's degree in emergency medical services or related field or considerable experience in emergency medical work at the paramedic level, including experience with social determinants of health and substance use recovery initiatives.
Special Requirements
Certification from the State at the Paramedic level of EMT.
Possession of a valid driver's license.
Certified as a Community Paramedic within two years of assignment to the role.
Possession of other specialized and specific certifications as specified by the department.

Stanly County offers an employee health clinic. Our on-site Advance Practice Provider will see employees and dependents covered on our county insurance plan free of charge. This service does not replace your primary healthcare providers but augments their care.
As a wellness benefit, employees have the option of joining the Stanly County YMCA. The County will pay half of the employee's membership.
Employee Assistant Program is offered through Atrium Health. Each employee will receive up to six (6) free sessions on a rolling year. Dependent children and/or spouse may also use the sessions.
RETIREMENT : Stanly County Employees will be a member of the N. C. Local Government Employees' Retirement System. Stanly County is required to withhold six percent (6%) from each paycheck and deposit it into the retirement system. The County also contributes to the plan.
