POSITION SUMMARY
The Housing Navigator is responsible for providing support and assistance to members and their families experiencing homelessness or at-risk of losing their housing.
WHERE YOULL WORK
This position is hybrid. You will work from home and service members in the field.
WHAT YOULL DO
- Research, establish, and maintain a list of available housing opportunities in the region
- Develop relationships with and become knowledgeable about local community resources including transitional housing, shelters, and permanent housing
- Develop individualized housing plan, communicate the plan and housing opportunities to the member to ensure effective service and all placements are consistent with the members overall care plan
- Assist members with their housing search, including applying for apartments, negotiating with potential landlords, filling out rental applications
- Assist members in understanding tenant rights and responsibilities
- Assist members with talking to their landlord by being a mediator if there is a disagreement or concern
- Conduct follow-up meetings with members after housing placement to assist with issues
- Maintain accurate documentation of service objectives, outcomes, and other member related information in the appropriate data systems
- Participate in collaborative team meetings to provide input, support, and brainstorm solutions to common areas of concern and develop best practices
- Understand and abide by all departmental and companywide policies and procedures
- Comply with all safety and injury prevention policies and regulations
- Utilize the HMIS: Homeless Management Information System to add members into housing portal
- Assist members in completing CES assessments; completing it with them on HMIS or directing them to a CES Access point where they can get the assistance to complete the CES assessment
- Assist members with in-person assessments or housing tenancy
- Perform other duties as assigned or required
WHO YOU ARE
- Fluent in English (written and verbal)
- Competent with computers, email, virtual platforms, Excel and other Microsoft Office based programs
- Excellent verbal and written communication skills, including the ability to convey and exchange information in a clear and concise manner
- Ability to identify problems and use logic and related information to develop and implement solutions
- Ability to work independently and carry out assignments to completion within the parameters of established policies and procedures
WHAT YOULL NEED
- Bachelors degree in healthcare, or 1-2 years of experience in healthcare, social work, public health, physiology, or other related field
- Minimum 1-2 years of relevant experience in social work, case management, working with unhoused and unsheltered individuals/families and veterans programs
- Experience in a social service setting with working knowledge of case management systems and planning techniques
- Current and valid Drivers License
- Proof of auto insurance
- Distraction-free home workspace with a secure internet connection

- Bilingual in Spanish a plus
- Prior experience working with HMIS
