What Youll Be Doing:

• Patient Care Coordination: Serve as the primary coordinator for patients, facilitating seamless collaboration across healthcare disciplines for well-rounded, effective care.

• Monitor and Communicate: Implement care plans, monitor patient conditions, and maintain open communication with the healthcare team and patients' families.

• Execute Medical Orders: Carry out physician and nursing orders with attention to detail, maintaining high standards of patient care.

• Discharge Planning: Work closely with healthcare team members to assess and coordinate patients' discharge needs.

Requirements:

• Experience: At least 1 year of experience as a Medical/Surgical RN.

• Skills: Proven ability in Medical/Surgical nursing with expertise in handling patient care documentation, assessment, and coordination.
