Positional Overview
Mary Kraft Staffing & HR is recruiting for a Care Manager RN on behalf of our client, a leading healthcare provider in St. Cloud, FL. This position offers full-time hours from 8am-5pm, Monday through Friday, and is an on-site role focused on supporting patient care transitions and coordinating patient needs.
In this role, you’ll join a dedicated team of 17 members, including three healthcare providers, who collaborate closely to manage patient care and support. You’ll play a vital part in overseeing transitions of care, triaging cases, scheduling, and maintaining thorough hospital tracking and follow-ups, with responsibilities that extend to coordinating with hospice and VITAS services

- Manage patient care throughout transitions, discharges, and hospital updates.
- Track and coordinate hospital information and follow-up requirements.
- Collaborate on care plans, ensuring seamless scheduling and patient transitions.
- Coordinate patient needs with hospice and VITAS services as needed.
- Serve as a key point of contact for patient care needs within the team.

- Case management experience.
- Active RN license.
- Strong Communication Skills – Ability to effectively communicate with patients, families, and healthcare providers to coordinate and ensure high-quality patient care.
- Attention to Detail – Meticulous in maintaining patient records, tracking follow-ups, and coordinating care to prevent errors and ensure continuity of care.
- Problem-Solving Abilities – Skilled in assessing situations quickly and developing effective solutions, especially when coordinating complex patient needs.
- Time Management – Effectively manages time and prioritizes tasks to meet the demands of a high-paced healthcare environment.
