Positional Overview
Mary Kraft HR is looking for a dedicated Case Manager RN to join our interdisciplinary healthcare team. In this role, you will enhance clinical outcomes and member satisfaction by managing care plans and conducting comprehensive evaluations of members' needs. You will collaborate with healthcare providers to tailor care strategies and advocate for patients throughout their healthcare journey. Role & Responsibility:
Tasks That Will Lead To Your Success
- Conduct thorough telephonic or face-to-face evaluations to assess members' individual health and social service needs.
- Develop, implement, and monitor individualized care plans in collaboration with members, families, and healthcare providers.
- Coordinate care for members with chronic illnesses and high-risk complications to ensure effective utilization of health benefits.
- Educate and support members and their families on available services and options for care delivery models.
- Facilitate referrals to community resources and social service programs to address members' broader needs.
- Document all assessments, care plans, and referrals accurately to maintain compliance and quality of care.
- Participate in interdisciplinary team meetings to share insights and contribute to safe transitions of care.
Skills & Experience:
Qualifications That Will Help You Thrive
- Current Registered Nurse (RN) license in the state of Texas.
- Bachelor’s degree in nursing (BSN) preferred, or an equivalent combination of education and experience.
- 2+ years of nursing experience, preferably in case management or a related healthcare setting.
- Strong understanding of healthcare delivery systems and interdisciplinary collaboration.
- Excellent communication and interpersonal skills for effective interactions with members, families, and healthcare teams.
- Proficiency in using electronic health record (EHR) systems and other healthcare management software.
