Program Description
The STARR program is designed to provide additional behavioral health services for individuals who have had contact with the criminal justice system. This program will be a collaboration between SF DPH, Felton Institute, SF Adult Probation Department and Salvation Army to provide additional residential treatment beds, low-threshold case management, and wraparound support services. Adults with Co-Occurring Treatment Disorders (i.e. SUD/SMI, unhoused, who have had contact with the criminal justice system) will be the population of focus.
Job Description
The Clinical Case Manager works as part of a multidisciplinary team and is responsible for coordinating client care within the team and with community partners. This includes evaluating the strengths and needs of clients; identifying treatment goals collaboratively with clients and team members. The Clinical Case Manager is expected to provide services to clients in both the office and in the community. The Clinical Case Manager reports directly to the STARR Program Manager.
- Provide case management for clients who desire mental health/substance use recovery and wellness services, crisis prevention/intervention within the context of a multi-disciplinary team.
- Develop and maintain good clinical relationships with clients.
- Utilize evidence-informed and evidence-based interventions to address client needs and goals.
- Collaborate with community partners acting as current access and intervention points; including jails, Adult Probation, and community social service providers.
- Meet documentation and other deadlines; complying with contractual deliverables.
- Perform administrative tasks within time-lines (chart completion, meeting attendance, etc.).
- Other duties and responsibilities may be assigned as required in order to meet program goals and objectives.
Minimum Qualifications
- MS/MSW/MA degree or higher in Psychology, Counseling or Social Sciences related fields. *New grads are encouraged to apply.*
- One year of field experience working with people currently and formerly incarcerated, people with serious mental illnesses, individuals with a dual diagnosis and/or the homeless population.
- Use of personal vehicle, proof of a valid and current California Driver’s License and current insurance along with a clean DMV record required.

- Strong analytical and problem-solving skills with meticulous attention to detail.
- Ability to work well independently and collaboratively with teams.
- Possess an understanding of and practice cultural sensitivity through open dialogue and self-exploration with diverse groups, while providing direct services.
- Some experience with outreach and providing clinical services to individuals in field settings (i.e., encampment, streets, psychiatric hospital, etc.) is preferred.
- Knowledge of Psychiatric Disorders as well as the ability to implement the following evidence-based models: Harm Reduction, Psychosocial Rehabilitation, Strength-based Case Management and Motivational Interviewing.
- Ability to build supportive and respectful working relationships with individuals diagnosed with a mental illness that instills hope and promotes self-determination, with a sensitivity to and understanding of the special needs of the clients.
- Experience in effectively intervening in crisis situations, with de-escalation techniques.
- Proficiency in Microsoft Office programs, systems, and platforms.
- Outstanding communication skills and a high degree of emotional intelligence, cultural humility, and the ability to build and maintain effective relationships with a wide variety of internal and external contacts.

FLSA Status: Exempt
Salary Range: $37,100 (Adjusted Part-Time salary)
