Job Summary

Within one of the nation’s most transformative universities, SJSU’s Student Wellness Center (SWC) fosters student opportunity in a revolutionary way by preparing students to maintain lifelong health and well-being. The RN Case Manager provides support for students accessing services across and beyond the Student Wellness Center by serving as a resource in managing care involving multiple care services. Reporting to the Director of Health Services, with support from SWC leadership, the RN Case Manager coordinates and monitors referrals within and outside of the SWC, including referral and liaison services with mental and physical health services, insurance providers, social services, etc. for students who need assistance navigating their care. The RN Case Manager will also respond to requests for care coordination that include referrals to external providers, documentation to be faxed/shared, and insurance research, including support for students who have no health insurance or are under-insured. As a Registered Nurse, the incumbent performs a variety of medical services for students at the Student Wellness Center under the general supervision. Working collaboratively to support student success is the bottom line purpose of this and all SWC positions.

Key Responsibilities

  • Develop and implement systems for follow-up with students to ensure continuity of care when utilizing multiple Student Wellness Center services
  • Support students accessing SWC services for a variety of care needs including, but not limited to mental health, eating disorders and/or disordered eating, sexual health needs, and gender affirming care
  • Assess and triage students referred to Health Services by other SJSU departments to educate on availability of services and appropriateness of services to meet student needs
  • Work with administrative support staff to ensure coordinated care across the SWC. Includes scheduling next appointment(s) as appropriate, referral follow-through, and observing high-need/high-risk care
  • Assess appropriateness of an external referral based on a student’s insurance and the care provider’s recommendations for further care
  • Collaborate with providers on determining appropriateness of other SJSU, community, or private resources when SWC internal resources are not available or appropriate to meet care needs
  • Make appropriate entries in patients’ electronic medical records
  • Administer immunizations, TB tests, and certain medications under the guidelines of standardized procedures
  • Build relationships with campus and community agencies to increase the referral network and ensure appropriate service-referrals for students

Knowledge, Skills & Abilities

  • Ability to effectively establish and maintain cooperative working relationships within a diverse multicultural environment
  • Ability to maintain confidentiality and knowledge of confidentiality requirements within healthcare and/or education settings (e.g. HIPAA, FERPA, etc.)
  • Knowledge of local/state community resources, including ability to navigate a higher education campus and the resources offered therein
  • Thorough knowledge of general methods, procedures, and practices of an outpatient medical facility
  • Ability to effectively apply nursing knowledge and demonstrate proper techniques and skills in performing nursing duties
  • Effective communication across a variety of venues. Working knowledge of English grammar, punctuation, and spelling
  • Working knowledge of electronic health record systems and ability to learn new systems and assess appropriate workflows within EHRs to manage patient cases
  • Ability to handle multiple work priorities, organize, and plan work and projects
  • Skill in motivational interviewing techniques and ability to conduct individual meetings. Must possess excellent customer service and public relations skills
  • Thorough knowledge of professional nursing principles, methods and procedures; anatomy and physiology; medical supplies and equipment used in nursing practice; and the uses and effects of medications and narcotics
  • Ability to analyze emergency situations and respond promptly and stay calm
  • Ability to take initiative and propose new processes and/or process improvements
  • Ability to problem solve and address confusion and frustration with multi-step processes

Required Qualifications

  • High School diploma or equivalent
  • Completion of a professional nursing program in an approved school of nursing or by additional preparation as approved by the California Board of Registered Nursing
  • Possess and maintain a valid license as a registered nurse in the State of California and certification in CPR/AED
  • Three (3) years of progressively responsible full-time professional nursing experience

Preferred Qualifications

  • One (1) year of experience providing case management in a nursing capacity
  • Demonstrated experience assisting patients navigating referrals and/or insurance resources
  • Demonstrated Experience working within an integrated health facility

Compensation

Classification: Registered Nurse III
Anticipated Hiring Range: $8,500/month - $9,300/month
CSU Salary Range: $5,285/month - $10,197/month
